【摘要】 目的评价超声在妊娠期附件扭转的诊断价值,总结超声诊断妊娠期附件扭转的声像图特征。方法回顾性分析2015年1月—2020年4月于医院经手术及病理证实的附件扭转的53例孕妇的临床及超声检查资料,观察超声诊断妊娠期附件扭转的诊断效能及声像图特点。结果超声诊断妊娠期附件扭转的符合率为84.9%,早、中、晚孕三组之间超声检出率差异无统计学意义(P=0.195)。53例患者超声显示扭转蒂部包块、患侧卵巢间质回声增强、患侧卵巢内血供减少/消失及腹/盆腔内游离液体等超声征象的比例分别为71.7%、22.6%、60.4%及32.1%。根据扭转主体的不同,妊娠期附件扭转可分为卵巢增大伴扭转型7例(13.2%)、输卵管肿物伴扭转型4例(7.5%)、卵巢肿物伴扭转型42例(79.2%)。结论超声诊断妊娠期附件扭转的诊断符合率较高,正确识别不同类型附件扭转的声像图特征及特异性超声征像是超声诊断的关键。更多还原

【Abstract】 Objective To evaluate the diagnostic value of ultrasound in the adnexal torsion in pregnancy and to summarize the sonographic characteristics of the adnexal torsion in pregnancy. Methods The clinical and ultrasound data of 53 pregnant women with adnexal torsion confirmed by surgery and pathology in our hospital from January 2015 to April 2020 were retrospectively analyzed, and the sonographic diagnostic efficiency and characteristics of adnexal torsion in pregnancy were observed. Results The overall detection rate of preoperative ultrasound for the adnexal torsion in pregnancy was 84.9%, and there was no statistical difference among the different gestational ages(P=0.195). The proportion of 53 ultrasonographic signs which includes torsion mass, ovarian edema, absence/decreased of blood flow in the ovary, free fluid in the abdomen/pelvic was 71.7%, 22.6%, 60.4% and 32.1% respectively. According to the different torsion subjects, there were 7 cases(13.2%) of ovarian enlargement with torsion, 4 cases(7.5%) of fallopian tube mass with torsion, and 42 cases(79.2%) of ovarian mass with torsion. Conclusion The detection rate of ultrasound in the adnexal torsion in pregnancy is high. It is critical to the diagnosis of adnexal torsion in pregnancy by correctly identifying the ultrasonic signs of different types of adnexal torsion.更多还原
