【摘要】 目的探讨内镜下黏膜剥离术(endoscopic mucosal dissection,ESD)在消化道早癌及癌前病变中的应用价值。方法选择我院2016年10月—2019年10月收治的100例消化道早癌及癌前病变患者作为本次研究入组对象,随机分为研究组和对照组,每组各50例,其中研究组采用ESD治疗,对照组采用内镜下黏膜切除术(endoscopic mucosal resection,EMR)治疗,对比两组治疗效果,对比两组完整切除率、手术时间、住院时间;对比两组术中出血、术后出血、术后穿孔情况。结果研究组完整切除率高于对照组(P <0.05),手术时间稍长于对照组(P <0.05),两组住院时间差异无统计学意义(P> 0.05)。两组术中出血、术后穿孔差异无统计学意义(P> 0.05)。两组均未发生术后出血(P> 0.05)。结论内镜下黏膜剥离术(ESD)在消化道早癌及癌前病变中的应用价值高,可完整切除肿物,避免外科手术,最大限度的保留正常组织器官,手术并发症少,效果好,治疗方法安全可靠。更多还原

【Abstract】 Objective To evaluate the value of endoscopic mucosal exfoliation(endoscopic mucosal dissection, ESD) in early carcinoma and precancerous lesions of digestive tract. Methods 100 patients with early carcinoma and precancerous lesions of digestive tract treated in our hospital from October 2016 to October 2019 were randomly divided into study group(n=50) and control group(n=50). The study group was treated with ESD and the control group was treated with endoscopic mucosal resection(endoscopic mucosal resection, EMR). The therapeutic effects of the two groups were compared, and the complete resection rate, operation time and hospital stay of the two groups were compared. The intraoperative bleeding, postoperative bleeding and postoperative perforation were compared between the two groups. Results The complete resection rate in the study group was higher than that in the control group(P < 0.05), and the operation time in the study group was slightly longer than that in the control group(P < 0.05). There was no significant difference in hospital stay between the two groups(P > 0.05). There was no significant difference in intraoperative bleeding and postoperative perforation between the two groups(P > 0.05). There was no postoperative bleeding in both groups(P > 0.05). Conclusion Endoscopic mucosal exfoliation(ESD) has high application value in early carcinoma and precancerous lesions of digestive tract. It can completely remove the tumor, avoid surgical operation, and maximize the preservation of normal tissues and organs. The operative complications are few, the effect is good, the treatment method is safe and reliable.更多还原
