2013~2019年单中心呼吸系统住院患儿流行病学及病原分析

【摘要】 目的了解2013~2019年间因呼吸系统疾病住院患儿的流行病学及病原学变化趋势,为更准确地诊断和治疗儿童呼吸道疾病提供参考。方法选取2013年7月至2019年6月在华中科技大学同济医学院附属同济医院儿童呼吸专科住院的11197例患儿作为研究对象,回顾性对比分析患儿年龄、性别、病原等资料。结果女性患儿占比由33.1%升至38.6%;患儿住院年龄呈逐年上升趋势,1岁以内患儿占比由24.2%降至10.7%,1~岁患儿由44.1%降至26.2%,2~岁由26.8%升至43.7%,5岁以上患儿由7.5%升至19.2%;主要诊断为肺炎及支气管炎,脓毒症诊断率呈逐年升高趋势;细菌检出率在2013~2014年间最高,达20.1%;病毒检出率最高在2018~2019年间,达32.7%;真菌检出率最高在2017~2018年间,达15.9%;非典型微生物检出率最高在2017~2018年间,达87.4%。结论因呼吸系统疾病住院的患儿中,女性患儿比例逐渐升高;患儿患病年龄也呈逐年升高趋势;近年来,细菌检出率呈下降趋势,而病毒检出率呈升高趋势,非典型病原体仍然是呼吸系统疾病住院患儿感染最主要的病原体。

【Abstract】 Objective To understand the epidemiological and etiological changes of hospitalized children with respiratory diseases in the past 6 years, so as to provide references for more accurate diagnosis and treatment of children with respiratory diseases.Methods from July 2013 to June 2019,11197 children hospitalized in Children’s Respiratory Department of Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology were selected, and the data of age, gender and pathogen were retrospectively analyzed and compared.Results The proportion of female children increased from 33.1% to 38.6% in recent years.The age of hospitalized children increased year by year, from 24.2% to 10.7% for children under 1 year old, from 44.1% to 26.2% for children aged 1-2 years old, from 26.8% to 43.7% for children aged 2-5 years old, and from 7.5% to 19.2% for children over 5 years old.The main diagnosis was pneumonia and bronchitis, and the diagnosis rate of sepsis gradually increased.The bacterial detection rate was the highest in 2013-2014,up to 20.1%.The highest detection rate was 32.7% in 2018-2019.The highest fungus detection rate was 15.9% in 2017-2018.The highest detection rate of atypical microorganisms was 87.4% in 2017-2018.Conclusion Among the hospitalized children with respiratory diseases, the proportion of female children is increased year by year.The age of the sick children is also increased year by year.In recent years, the detection rate of bacteria is decreased, while the detection rate of virus is on the rise.
