【摘要】 目的:探讨P65和P16在皮肤恶性黑色素瘤(CMM)患者中的表达及其临床意义。方法:采用免疫组化检测89例CMM患者中P65、P16的表达情况,统计患者的临床资料分析其表达与临床特征的相关性,采用Kaplan-Meier曲线对随访数据进行预后分析。结果:(1) CMM患者中P65的表达明显高于色素痣患者(χ2=31.190,P <0.001),P16的表达明显低于色素痣患者(χ2=22.379,P <0.001);(2) P65的表达与肿瘤浸润层次(χ2=11.796,P=0.003)、淋巴结转移(χ2=5.920,P=0.015)和远处转移(χ2=5.481,P=0.019)呈正相关,P16的表达与远处转移(χ2=4.895,P=0.027)呈负相关;(3) P65表达阳性的患者,中位总体生存期明显短于表达阴性的患者(22.0月vs 28.7月,P=0.019),P16的表达与总体生存期无关;(4)多因素生存分析显示P65表达阳性和有远处转移是CMM患者预后的独立危险因素。结论:P65、P16可能在CMM的发生和发展中起重要作用,P65也可能对CMM患者的预后有一定的指导意义。更多还原

【Abstract】 Objective: To investigate the expression and significance of P65 and P16 in cutaneous malignant melanoma patients. Methods: SP immunohistochemistry was used to detect the expression of P65 and P16 in 89 cutaneous malignant melanoma patients. The clinical data were collected to analyze the relationship between the expression and patients clinical characteristics. Kaplan-Meier curves were used to conduct survival analysis of follow-up data. Results:( 1) The expression of P65 in cutaneous malignant melanoma patients was significantly higher than that in patients with pigmented nevus( χ2= 31. 190,P < 0. 001),and the expression of P16 in cutaneous malignant melanoma patients was significantly lower than that in patients with pigmented nevus( χ2=22. 379,P < 0. 001).( 2) There was a positive correlation between the expression of P65 and tumor infiltration anatomical hierarchy( χ~2= 11. 796,P = 0. 003),lymph node metastasis( χ~2= 5. 920,P = 0. 015) and distant metastasis( χ~2= 5. 481,P = 0. 019),and there was a negative correlation between the expression of P16 and distant metastasis( χ~2= 4. 895,P = 0. 027).( 3) The median overall survival time of patients with positive expression of P65 was significantly shorter than that with negative expression( 22. 0 months vs 28. 7 months,P = 0. 019). The expression of P16 had no relationship with overall survival time.( 4) Multivariate survival analysis showed that positive expression of P65 and distant metastasis were independent risk factors for cutaneous malignant melanoma patients. Conclusion: P65 and P16 may play a important role in the occurrence and development of cutaneous malignant melanoma. P65 may be helpful for the prognosis of patients with cutaneous malignant melanoma.更多还原
