【摘要】 目的:观察分析加味柴胡龙骨牡蛎汤联合隔药灸治疗抑郁症的临床疗效。方法:选取河南省洛阳荣康医院2018年5月至2020年1月收治的肝郁气滞型抑郁症患者106例,随机分为观察组和对照组,各53例。对照组给予盐酸帕罗西汀片治疗,观察组在对照组治疗的基础上给予加味柴胡龙骨牡蛎汤+隔药灸治疗。观察分析两组患者的中医证候积分、汉密尔顿抑郁量表(HAMD)评分、生活质量评价量表(short form 36 questionaire,SF–36)评分、脑源性神经营养因子(BDNF)、5–羟色胺(5–HT)、白细胞介素–6(IL–6)水平及复发情况。结果:观察组患者的临床总有效率为92.45 %,明显高于对照组的73.58 %,差异具有统计学意义(P<0.05)。治疗后,两组患者中医证候积分和HAMD评分均明显降低,SF–36评分明显升高,且观察组患者中医证候积分和HAMD评分低于对照组,SF–36评分高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组患者血清BDNF和5–HT水平均明显升高,IL–6水平明显降低,且观察组患者血清BDNF和5–HT高于对照组,IL–6低于对照组,差异具有统计学意义(P<0.05)。观察组治疗结束后第6个月复发率为10.20 %,低于对照组的30.77 %,差异具有统计学意义(P<0.05)。结论:加味柴胡龙骨牡蛎汤联合隔药灸治疗抑郁症疗效较好,可有效缓解抑郁状态,改善单胺类递质代谢,促进神经元恢复,降低复发率,提高生活质量。更多还原

【Abstract】 Objective To observe and analyze the clinical efficacy of Jiawei Chaihu Longgu Muli Decoction combined with medicinal moxibustion in the treatment of depression.Methods A total of 106 patients with liver-stagnation and qi stagnation type depression who were admitted to Luoyang Rongkang Hospital from May 2018 to January 2020 were randomly divided into observation group and control group,with 53 cases in each group.The control group was given paroxetine hydrochloride tablets,and the observation group was given modified Chaihu Longgu muli Decoction + medicated moxibustion on the basis of the treatment of the control group.The TCM syndrome score,Hamilton Depression Scale (HAMD) score,quality of life evaluation scale (short form 36 questionaire,SF-36) score,brain-derived neurotrophic factor (BDNF),5-hydroxytryptamine (5-HT),interleukin-6 (IL-6) and recurrence were observed and compared.Results The total clinical effective rate of patients in the observation group was 92.45%,which was significantly higher than 73.58% in the control group.The difference was statistically significant (P < 0.05).After treatment,the TCM syndrome score and HAMD score of the two groups of patients were significantly reduced,and the SF-36 score was significantly increased,these indexes in the observation group was better than those in the control group,and the differences were all statistically significant (P < 0.05).After treatment,serum BDNF and 5-HT levels of the two groups were significantly increased,and IL-6 levels were significantly reduced,and these indexes in the observation group was better than those in the control group,the differences were statistically significant (P < 0.05).The recurrence rate in the observation group at the 6th month after treatment was 10.20%,which was lower than that of the control group,30.77%,the difference was statistically significant (P < 0.05).Conclusion Modified Chaihu Longgumuli Decoction combined with medicine-partitioned moxibustion has a good effect on depression.It can effectively relieve depression,improve monoamine transmitter metabolism,promote neuronal recovery,reduce recurrence rate and improve quality of life.更多还原
