微视频指导下早期康复训练对心脏瓣膜置换术患者心脏功能呼吸功能及运动耐力的影响
Effect of early rehabilitation training under micro-video guidance on cardiac function,respiratory function, and exercise endurance in patients who underwent heart valve replacement surgery
【摘要】 目的探讨微视频指导下早期康复训练对心脏瓣膜置换术患者心脏功能、呼吸功能及运动耐力的影响。方法将我院2018年7月至2020年10月107例心脏瓣膜置换术患者分组,按照随机数字表法分为对照组53例,术后予以呼吸功能训练,观察组54例在对照组基础上增加微视频指导下早期康复训练,观察2组心脏功能、呼吸功能、运动耐力及心脏不良事件。结果干预后,观察组左心室射血分数(LVEF)、二尖瓣舒张早末期血流峰值比(E/A)、冠状动脉血流储备分数(FFR)水平均较对照组高(P<0.05);观察组最大通气量(MVV),血氧饱和度(SpO2)、动脉血氧分压(PaO2)水平较对照组高,心率、呼吸频率水平较对照组低(P<0.05);观察组6 min步行试验距离(6MWT)较对照组远,FMA评分较对照组高(P<0.05);观察组心脏不良事件发生率(4%)较对照组(17%)低(P<0.05)。结论微视频指导下早期康复训练可通过改善心脏瓣膜置换术患者呼吸功能,强化运动耐力,提高心脏功能,减少术后心脏不良事件发生。
【Abstract】 Objective To explore the effect of early rehabilitation training guided by micro-video on cardiac function, respiratory function, and exercise endurance in patients undergoing heart valve replacement. Methods A total of 107 patients who underwent heart valve replacement surgery in our hospital between July 2018 and October 2020 were divided into two groups using the random number table method. Fifty-three patients in the control group received postoperative respiratory function training and 54 patients in the observation group increased early rehabilitation training under micro-video guidance based on the control group′ s training. The cardiac function,respiratory function, exercise endurance, and adverse cardiac events of the two groups were observed. Results After the intervention, the left ventricular ejection fraction(LVEF),peak mitral valve early and end diastolic blood flow ratio(E/A),and coronary blood flow reserve(FFR) levels of the observation group were higher than those of the control group(P <0.05). The maximum ventilation(MVV),blood oxygen saturation(SpO2),and arterial partial pressure of oxygen(PaO2)levels of the observation group were higher than those of the control group. In contrast,the heart rate(HR)and respiratory rate(R)of the observation group were lower than those of the control group(P<0.05). The 6-min walk test distance(6 MWT)of the observation group was greater than that of the control group,and the FMA score was higher than that of the control group(P<0.05). The incidence of cardiac adverse events in the observation group was 4% which was lower than the 17% of the control group(P <0.05). Conclusion Early rehabilitation training under micro-video guidance can improve respiratory function,lengthen exercise endurance,improve cardiac function,and reduce adverse cardiac events after surgery in patients who underwent heart valve replacement.
