气道表面液糖浓度对不同病原体致慢性阻塞性肺疾病急性加重的诊断价值
Diagnostic value of airway surface luqild glucose in acute exacerbation of chronic obstructive pulmonary disease caused by different pathogens
【摘要】 目的探讨气道表面液糖浓度(文章主要指痰液中的葡萄糖浓度)对不同病原体致慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)急性加重的诊断价值。方法收集邵阳市中心医院呼吸与危重症医学科2019年12月1日到2021年3月1日临床上诊断为COPD急性加重期患者80例,根据临床症状、胸部影像学、实验室检查(痰培养、血清学检测、抗原检测、分子生物学检测)等结果将患者分为细菌感染组50例和真菌感染组30例,并选择同期入院的健康体检者40例作为痰培养阴性组。收集患者及健康体检者入院当天清晨漱口后第一口深部痰液,采用己糖激酶法测定其葡萄糖浓度,对3组患者气道表面液(airway surface liquid, ASL)糖浓度进行比较,并分析与痰培养结果的相关性。结果 3组患者均完成气道表面液糖浓度测定,其中细菌感染组气道表面液糖浓度最高,痰培养阴性组气道表面液糖浓度最低,差异具有统计学意义(P<0.05)。结论气道表面液糖浓度对不同病原体所致COPD急性加重有一定的鉴别诊断意义。
【Abstract】 Objective To explore the diagnostic value of airway surface luqiud glucose(mainly referred to the glucose concentration in sputum in this paper) in the diagnosis of acute exacerbation of chronic obstructive pulmonary disease(COPD) caused by different pathogens. Methods A collection of 80 patients with COPD diagnosed clinically in the Department of Respiratory Medicine of our hospital from December 1, 2019 to March 1, 2021 were collected. Aaccording to clinical symptoms, chest X-ray examination, laboratory examination(sputum culture, serological testing, antigen testing, molecular biology testing), and other results were divided into a bacterial infection group of 50 cases and a fungal infection group of 30 cases, and selected 40 healthy subjects who were admitted to our hospital during the same period as the sputum culture negative group. The first deep sputum was collected from patients and healthy subjects after gargling with water on the day of admission, and the glucose concentration was measured by hexokinase method. The glucose concentration in airway surface fluid of three groups was compared and the correlation with sputum culture results was analyzed. Results All 120 people completed the determination of airway surface luqiud glucose; the bacteria group had the highest sputum airway surface luqiud glucose, and the negative group had the lowest airway surface luqiud glucose, with statistically significant difference(P<0.05). Conclusion Airway surface luqiud glucose has certain significance in the differential diagnosis of acute exacerbation of COPD caused by different pathogens.
