MRI定量评估先天性膈疝胎儿预后的受试者工作特征曲线分析

【摘要】 目的 运用受试者工作特征(ROC)曲线分析确定可用于判断孤立性先天性膈疝(CDH)胎儿预后的MRI观察值/预期值胎儿肺容积(o/e FLV)界定值。方法 选取孕18~38周的孤立性CDH胎儿98例。应用新的适用于国内的FLV预期值回归方程来测算孤立性CDH胎儿的MRI o/e FLV值。按产后是否存活分组观察o/e FLV情况。采用ROC曲线分析确定孤立性CDH胎儿产后能否生存的o/e FLV界定值及敏感度、特异度。结果 产后存活组和死亡组的o/e FLV平均值分别为52.59%±15.08%和29.57%±8.52%。ROC曲线分析显示最佳MRI o/e FLV界定值为37.82%,灵敏度为86.1%,特异度为88.5%。结论 MRI o/e FLV界定值的确定为判断孤立性CDH胎儿预后提供了依据,有助于孤立性CDH胎儿的产前咨询与管理。

【Abstract】 Objective To determine the cut-off value of MRI observed/expected fetal lung volume(o/e FLV) for the prognosis of fetuses with isolated congenital diaphragmatic hernia(CDH) by receiver operating characteristic(ROC) curve analysis.Methods Ninety-eight fetuses with isolated CDH at 18-38 gestation weeks were collected.The new domestic FLV expected value regression equation was used to calculate the MRI o/e FLV value of isolated CDH fetuses.Fetuses with isolated CDH were grouped according to survival or death after delivery,and o/e FLV values of two groups were observed respectively.ROC curve analysis was used to determine the postpartum survival cut-off value of o/e FLV and its sensitivity and specificity.Results The mean values of o/e FLV in postpartum survival and death groups were 52.59% ±15.08% and 29.57 % ± 8.52%,respectively.ROC curve analysis showed that the best MRI o/e FLV cut-off value was 37.82%,the sensitivity was 86.1 %,and the specificity was 88.5%.Conclusion Determined MRI o/e FLV cut-off value provides a basis for evaluating the prognosis of isolated CDH fetuses,which will be helpful for prenatal consultation and management of isolated CDH fetuses.
