手术联合术后适形调强放疗治疗早期颅内胶质瘤的疗效及安全性

【摘要】 目的评价手术联合术后适形调强放疗治疗早期颅内胶质瘤的疗效及安全性。方法选取70例早期颅内胶质瘤患者作为研究对象,随机将其分为2组。对照组35例,给予手术联合普通放疗治疗;研究组35例,给予手术联合术后适形调强放疗治疗。观察2组患者的临床疗效、治疗安全性、肿瘤复发率、总生存率、无疾病进展生存率情况。结果研究组总有效率为97.14%;对照组总有效率为70.59%,2组总有效率差异具有统计学意义(P <0.05)。研究组患者Ⅰ~Ⅱ度消化道症状、骨髓抑制、放射性损伤发生率与对照组对比,差异无统计学意义(P> 0.05);Ⅲ~Ⅳ度上述指标比较,研究组明显低于对照组,差异有统计学意义(P <0.05)。治疗后肿瘤复发率研究组明显低于对照组,差异具有统计学意义(P <0.05)。研究组术后1~3年总生存率、无疾病进展生存率明显优于对照组,差异具有统计学意义(P <0.05)。结论手术联合术后适形调强放疗治疗早期颅内胶质瘤的疗效显著,安全可靠,值得临床推广和应用。

【Abstract】 Objective To evaluate the efficacy and safety of surgery combined with postoperative conformal intensitymodulated radiotherapy( IMRT) for early intracranial glioma. Methods 70 patients with early intracranial gliomas were randomly divided into 2 groups: the control group( 35 cases) and the study group( 35 cases). The clinical efficacy,treatment safety,tumor recurrence rate,overall survival rate and progression free survival rate of the 2 groups were observed. Results The total effective rate was 97. 14% in the study group and 70. 59% in the control group,the difference was statistically significant( P < 0. 05); Ⅰ~ Ⅱ gastrointestinal symptoms,bone marrow suppression,incidence of radiation injury of the 2 groups had no significant difference( P > 0. 05); Ⅲ ~ Ⅳ gastrointestinal symptoms,bone marrow suppression,incidence of radiation injury in the study group was significantly lower than the control group( P < 0. 05); Rate of tumor recurrence after treatment in the study group was significantly lower than the control group( P < 0. 05); The 1-3-year overall survival rate and disease-free progression survival rate in the study group were significantly better than those in the control group( P < 0. 05). Conclusion Surgery combined with postoperative conformal intensity-modulated radiotherapy is effective in treating early intracranial glioma.
