PD-L1表达与胰腺癌发生发展的相关性meta分析

【摘要】 目的采用meta分析评价PD-L1的表达与胰腺导管腺癌(pancreatic ductal adenocarcinoma,PDAC)预后及临床病理特征的关系。方法检索Pub Med、EMBASE、Medline、中国知网、万方数据资源库和Cochrane临床对照试验中心注册数据库(CENTRAL)中,关于PD-L1蛋白表达与胰腺导管腺癌患者临床特征和总生存情况关系相关的实验研究,采用Revman 5.3软件进行meta分析。结果共纳入16项研究(包括2085例胰腺导管腺癌患者),汇总分析显示,PDL1高表达者的死亡风险高(HR=1.61,95%CI=1.37~1.89,P <0.00001)。PD-L1表达水平与性别、糖尿病、肿瘤生长部位、远处转移(M)无明显关联,而与肿瘤侵润程度、组织分化程度、神经侵润、淋巴转移、吸烟有显著相关性。结论PD-L1高表达是胰腺导管腺癌预后危险因素,在肿瘤组织分化程度差、T3和T4期、神经侵润阳性、淋巴转移阳性、吸烟的胰腺导管腺癌患者中常发生。

【Abstract】 Objective To study the relation between the expression of high PD-L1 and prognosis and clinicopathological features of patients with pancreatic cancer cell carcinoma. Methods Electronic databases,such as Pub Med,Embase,Medline,CNKI,CJFD and Cochrane library,were searched to identify studies evaluating PD-L1 expression and overall survival( OS) in these patients. Rev Man 5. 3 software was used for meta-analtsis. Results 16 studies that involved 2085 patients were analyzed.The results showed that the PD-L1 protein’s high expression was leading to a poor overall survival rate( hazard ratio,HR = 1. 61,95% CI = 1. 37 ~ 1. 89,P < 0. 00001). But it was not related to gender,diabetes,tumor growth site and distant metastasis( M-vs M+). However,a high expression of PD-L1 was significantly correlated to the low differentiation,tumor infiltration depth,neural invasion,lymph node metastasis and smoking. Conclusion A high level of PD-L1 protein expression might be a risk prognostic factor for patients with pancreatic ductal adenocarcinoma. PD-L1 positive expression is more common in patients with low differentiation,positive tumor invasion,lymphatic invasion positive,smoking pancreatic cancer.
