肺表面活性物质联合机械通气治疗新生儿呼吸窘迫的疗效及CRP水平分析

【摘要】 目的评价肺表面活性物质联合机械通气治疗新生儿呼吸窘迫综合征(NRDS)的疗效及对C反应蛋白(CRP)水平的影响。方法按随机数表法将2019年11月—2021年3月就诊于该院的116例NRDS患儿分为两组,对照组(58例)接受机械通气治疗,研究组(58例)实施机械通气+肺表面活性物质治疗。对比两组疗效、机械通气相关指标、呼吸力学、血清炎症指标及并发症。结果研究组治疗总有效率(94.83%)高于对照组(81.03%),差异有统计学意义(χ~2=5.199,P=0.023);研究组机械通气时间(4.03±0.89)d、给氧时间(6.34±1.24)d、呼吸暂停次数(2.41±0.33)次、住院时间(22.58±2.34)d优于对照组,差异有统计学意义(t=9.848、8.081、21.798、6.942,P<0.001);研究组治疗后气道阻力、内源性呼气末正压及CRP、高迁移率族蛋白-1(HMGB-1)、巨噬细胞移动抑制因子(MIF-1)、转化生长因子-β1(TGF-β1)水平比对照组低,静态顺应性比对照组高,差异有统计学意义(t=13.201、5.421、8.866、16.791、26.776、2.975、8.445,P<0.05)。结论NRDS患儿接受肺表面活性物质联合机械通气治疗的效果明显,可缩短机械通气时间,改善呼吸力学,抑制炎症因子。

【Abstract】 Objective To evaluate the efficacy of pulmonary surfactant combined with mechanical ventilation in the treatment of neonatal respiratory distress syndrome(NRDS) and the effect of C-reactive protein(CRP) level. Methods According to the random number table method, 116 children with NRDS who were treated in the hospital from November 2019 to March 2021 were divided into two groups.The control group(58 cases) received mechanical ventilation, and the study group(58 cases) received mechanical ventilation + pul-monary surfactant treatment. The efficacy, mechanical ventilation-related indicators, respiratory mechanics, serum inflammation indi-cators, and complications were compared between the two groups. Results The total effective rate of the study group(94.83%) was higher than that of the control group(81.03%), the difference was statistically significant(χ~2=5.199, P=0.023). Mechanical ventilation time(4.03±0.89) d, oxygen time(6.34±1.24) d, apnea times(2.41±0.33) times, hospital stay(22.58±2.34) d in the study group were better than in the control group, the difference was statistically significant(t=9.848, 8.081, 21.798, 6.942, P<0.001). After treatment,the levels of airway resistance, endogenous positive end-expiratory pressure, CRP, high mobility group protein-1(HMGB-1),macrophage mobility inhibitory factor(MIF-1) and transforming growth factor-β1(TGF-β1) in the study group were lower than those in the control group. The static compliance was higher than that of the control group, the difference was statistically significant( t=13.201, 5.421, 8.866, 16.791, 26.776, 2.975, 8.445, P<0.05). Conclusion The effect of pulmonary surfactant combined with mechanical ventilation on NRDS is obvious, which can shorten the time of mechanical ventilation, improve respiratory mechanics and inhibit inflammatory factors.
