Supreme喉罩在侧卧位手术中的临床应用研究
Clinical Application of Supreme Laryngeal Mask in Lateral Decubitus Surgery
【摘要】 目的探讨Supreme喉罩在侧卧位全麻手术中的临床应用情况。方法将2019年4月—2020年4月期间在该院开展侧卧位全麻手术患者作为研究对象,共92例,以SAS软件的随机数生成器模块将患者随机分成两组,其中对照组在气管导管通气下完成侧卧位全麻手术,观察组在Supreme喉罩通气下完成侧卧位全麻手术。对比两组患者的手术开展情况及术后恢复情况。结果观察组手术时间、麻醉时间、复苏室停留时间、失血量分别为(102.36±24.25)min、(135.14±30.49)min、(39.18±9.14)min、(167.15±45.16)m L。对照组手术时间、麻醉时间、复苏室停留时间、失血量分别为(105.16±25.47)min、(132.47±31.69)min、(38.47±9.22)min、(162.48±44.21)mL,差异无统计学意义(t=0.540、0.412、0.371、0.501,P>0.05);观察组住院时间(6.39±1.62)d,短于对照组(7.58±1.86)d,差异有统计学意义(t=3.272,P<0.05)。观察组术中低体温、躁动、低血压等发生率分别为2.17%、4.35%、2.17%;对照组术中低体温、躁动、低血压等发生率分别为2.17%、2.17%、4.35%,差异无统计学意义(χ~2=1.000、1.000、1.000,P>0.05)。观察组胃肠道反应、咽喉部不适发生率分别为6.52%、0.00%;对照组胃肠道反应、咽喉部不适发生率分别为23.91%、15.22%,差异有统计学意义(χ~2=4.128、5.566,P<0.05)。结论 Supreme喉罩能够满足侧卧位全麻手术的开展需要,并且在麻醉效果上与常规气管插管无显著差异,但患者术后恢复更快,胃肠道反应、咽喉部不适发生率更低。
【Abstract】 Objective To explore the clinical application of Supreme laryngeal mask in general anesthesia in lateral position. Methods A total of 92 patients who underwent general anesthesia in lateral position in the hospital from April 2019 to April 2020 were taken as the research object. The patients were randomly divided into two groups using the random number generator module of the SAS software. In the control group, the lateral position general anesthesia operation was completed under tracheal tube ventilation, and the ob-servation group was completed lateral position general anesthesia operation under supreme laryngeal mask ventilation. The operation development and postoperative recovery of the two groups of patients were compared. Results The operation time, anesthesia time, stay time in the resuscitation room, and blood loss in the observation group were(102.36 ±24.25) min,(135.14±30.49) min,(39.18±9.14) min, and(167.15±45.16) mL respectively. In the control group, the operation time, anesthesia time, stay time in the resuscitation room, and blood loss were(105.16±25.47) min,(132.47±31.69) min,(38.47±9.22) min, and(162.48±44.21) m L, respectively, with no statistically significant differences(t=0.540, 0.412, 0.371, 0.501,P>0.05); the hospital stay in the observation group was(6.39±1.62)d, which was shorter than the control group(7.58±1.86) d, the difference was statistically significant(t=3.272,P<0.05). The incidence of hypothermia, restlessness, and hypotension in the observation group were 2.17%, 4.35%, and 2.17%, respectively; the incidence of hypothermia, restlessness, and hypotension in the control group were 2.17%, 2.17%, and 4.35%, respectively. The difference was not statistically significant(χ2=1.000, 1.000, 1.000,P>0.05). The incidence of gastrointestinal reaction and throat discomfort in the observation group were 6.52% and 0.00%, respectively; the incidence of gastrointestinal reaction and throat discomfort in the control group were 23.91% and 15.22%, respectively, and the difference was statistically significant(χ2=4.128, 5.566,P<0.05). Conclusion Supreme laryngeal mask can meet the development needs of general anesthesia in lateral position, and the anesthesia effect is not significantly different from conventional tracheal intubation, but the patient recovers faster after surgery, and the incidence of gastrointestinal reactions and throat discomfort is lower.
