探讨血栓弹力图在创伤性休克患者早期诊断凝血功能变化中的应用价值

【摘要】 目的分析创伤性休克患者早期凝血功能变化诊断中应用血栓弹力图检测的作用。方法随机数字表法将2017年10月—2020年2月共120例创伤性休克患者分为两组,60例接受常规检测,60例在常规检测的基础上再接受血栓弹力图检测,分别视作对照组、观察组,比较两组凝血功能的检测结果。结果观察组对凝血功能障碍的诊断率为93.33%,高于对照组80.00%,差异有统计学意义(χ~2=4.615,P<0.05);凝血功能障碍诊断用时为(35.18±11.46)s短于对照组(60.28±21.04)s,住院时间为(10.28±3.32)d短于对照组(15.86±3.81)d,差异有统计学意义(t=8.115、8.553,P<0.05);观察组患者血浆、血小板、红悬液、冷沉淀使用量均少于对照组,差异有统计学意义(t=8.596、27.439、9.948、21.554,P<0.05);观察组输血24 h后FIB水平高于对照组,PT、TT、APTT水平低于对照组,差异有统计学意义(P<0.05)。结论血栓弹力图可更为实时地检测创伤性休克患者早期凝血功能变化,为临床创伤性休克的治疗提供更有用的指导信息。

【Abstract】 Objective To analyze the role of thromboelastography in the diagnosis of early coagulation changes in patients with traumatic shock. Methods A total of 120 patients with traumatic shock from October 2017 to February 2020 were divided into two groups by random number table method. 60 patients received routine testing, and 60 patients received thromboelastogram testing on the basis of routine testing, as a control group and an observation group, the coagulation function test results of the two groups were compared.Results The diagnosis rate of coagulation dysfunction in the observation group was 93.33%, which was higher than 80.00% in the control group. The difference was statistically significant(χ~2=4.615, P<0.05); the diagnosis time for coagulation dysfunction was(35.18±11.46) s, which was shorter than the control group(60.28±21.04) s, the hospitalization time was(10.28±3.32) d, which was shorter than the control group(15.86±3.81) d, the difference was statistically significant(t=8.115, 8.553, P<0.05). The consumption of plasma,platelets, red suspension, and cryoprecipitate in the obserration were all less than those of the control group, and the difference was statistically significant(t=8.596, 27.439, 9.948, 21.554, P<0.05); the FIB level of the observation group was higher than the control group after 24 hours of blood transfusion in the group, the levels of PT, TT, and APTT were lower than those of the control group, and the difference was statistically significant(P<0.05). Conclusion Thromboelastography can detect the early changes of coagulation function in patients with traumatic shock in more real-time, and provide more useful guidance information for the treatment of clinical traumatic shock.
