电针联合高压氧治疗不完全性脊髓损伤的疗效观察
Efficacy Observation of Electroacupuncture Combined with Hyperbaric Oxygen for Incomplete Spinal Cord Injury
【摘要】 目的观察电针联合高压氧治疗不完全性脊髓损伤的临床疗效。方法将90例确诊为不完全性脊髓损伤患者按随机数字表法分为电针组、高压氧组和联合组,每组30例。3组均进行常规治疗(西药加康复训练),电针组在常规治疗基础上加用电针治疗,高压氧组在常规治疗基础上加用高压氧治疗,联合组在常规治疗基础上加用电针和高压氧治疗。观察3组治疗前后美国脊柱损伤协会(ASIA)运动及感觉功能评分、改良Barthel指数(MBI)评分、外周血脑源性神经生长因子(BDNF)水平,并比较3组临床疗效及不良反应发生率。结果联合组总有效率高于高压氧组及电针组(P<0.05)。3组治疗后ASIA运动及感觉功能评分、MBI评分、BDNF水平均高于同组治疗前(P<0.01),且联合组治疗后各项指标均高于同期高压氧组及电针组(P<0.01)。联合组各项不良反应发生率均低于高压氧组和电针组,但差异无统计学意义(P>0.05)。结论电针联合高压氧治疗不完全性脊髓损伤疗效显著。
【Abstract】 Objective To observe the clinical efficacy of electroacupuncture(EA) plus hyperbaric oxygen in treating incomplete spinal cord injury(ISCI). Method Ninety patients diagnosed with ISCI were divided into an EA group, a hyperbaric oxygen group and an integrated group using the random number table method, with 30 cases in each group. The three groups all received conventional treatment(Western medication and rehabilitation training). In addition, the EA group was given EA treatment; the hyperbaric oxygen group received hyperbaric oxygen; the integrated group received both EA and hyperbaric oxygen. The motor and sensory function scores of American Spinal Injury Association(ASIA), Modified Barthel Index(MBI) score, brain-derived neurotrophic factor(BDNF) level in peripheral blood and adverse reaction rate in the three groups were observed before and after treatment. The clinical efficacy was also compared among the three groups. Result The total effective rate was higher in the integrated group than in the hyperbaric oxygen group and EA group(P<0.05). The motor and sensory function scores of ASIA, MBI score and BDNF level increased after treatment in the three groups(P<0.01), and were higher in the integrated group than in the hyperbaric oxygen group and EA group(P<0.01). The adverse reaction rates were lower in the integrated group than in the hyperbaric oxygen and EA groups, but without statistical significance(P>0.05). Conclusion EA combined with hyperbaric oxygen can produce significance efficacy in treating ISCI.
