17β-HSD-1、CD138和CD105在子宫内膜息肉组织中的表达及临床意义

【摘要】 目的:探讨子宫内膜息肉(EP)组织中17β-羟类固醇脱氢酶1(17β-HSD-1)、膜硫酸乙酰肝素蛋白多糖配体蛋白聚糖-1(CD138)和血管生成标志因子内皮糖蛋白(CD105)的表达及其与临床参数的关系。方法:收集安徽省第二人民医院收治的EP患者39例,取其EP组织样本(增生期19例、分泌期20例)作为息肉组,及其息肉旁内膜组织样本作为息肉旁组,另选择同期因不孕症或异常子宫出血行宫腔镜检查的40例正常子宫内膜组织作为正常对照组(增生期21例,分泌期19例)。采用免疫组织化学方法测定17β-HSD-1、CD138和CD105在3组内膜组织中增生期及分泌期的表达情况。结果:17β-HSD-1、CD138、CD105在息肉组中的阳性表达率明显高于息肉旁组和正常对照组,差异均有统计学意义(P<0.05)。息肉组增生期CD138和CD105的阳性表达率显著低于分泌期,差异有统计学意义(P<0.05),而17β-HSD-1的阳性表达率在增生期和分泌期差异无统计学意义(78.9%vs 80.0%,P>0.05)。息肉组异常子宫出血患者CD105的阳性表达率显著低于无异常子宫出血者,差异有统计学意义(62.1%vs 90.0%,P<0.05)。17β-HSD-1和CD138(r=0.070)、17β-HSD-1和CD105(r=0.010)、CD138和CD105(r=0.010)在EP组织增生期及分泌期的阳性表达间均无相关性(P>0.05)。结论:EP组织中17β-HSD-1、CD138和CD105均呈现过表达,这种异常表达可能与EP的发生密切相关,其有可能参与了EP的形成,可能作为EP诊断的参考指标。

【Abstract】 Objective:To investigate the expressions of 17β-hydroxysteroid dehydrogenase 1(17β-HSD-1),membrane heparan sulfate proteoglycan ligand proteoglycan-1(CD138)and the marker factor of angiogenesis(CD105)in endometrial polyps(EP)tissues and their relationship with clinical parameters.Methods:Collect 39 patients with endometrial polyps diagnosed in Anhui NO.2 Provincial People′s Hospital, take their endometrial polyp tissue samples(19 cases in the proliferative phase, 20 cases in the secretory phase)as the polyp group, and the parapolyp endometrial tissue samples as the parapolyp group, and select the same period 40 cases of normal endometrial tissue undergoing hysteroscopy due to infertility or abnormal uterine bleeding served as the control group(21 cases in the proliferative phase, 19 cases in the secretory phase).Immunohistochemical methods were used to determine the positive expression of 17β-HSD-1,CD138 and CD105 in the proliferative and secretory phases of endometrial polyps and normal endometrium.Results:The positive expression rates of 17β-HSD-1,CD138,and CD105 in the polyp group were significantly higher than those in the parapolyp group and the normal group(P<0.05),but there was no significant difference between the parapolyp group and the normal group(P>0.05).The positive expression rates of CD138 and CD105 in the proliferation period of the polyp group were lower than those in the secretion period.The differences were statistically significant(P<0.05),but the positive expression rates of 17β-HSD-l in the proliferation period of the polyp group was no significant difference than those in the secretion period(78.9% vs. 80.0%,P>0.05).The positive expression rate of CD105 in polyps with abnormal uterine bleeding was lower than those without abnormal uterine bleeding, and the difference was statistically significant(62.1% vs. 90.0%,P<0.05).There was no significant correlation between the positive expression of the three factors in the proliferation and secretion phases of the polyp group(P>0.05).Conclusions:17β-HSD-1,CD138 and CD105 are all overexpressed in endometrial polyps.This abnormal expression may be closely related to the occurrence of endometrial polyps, which may be involved in the formation of endometrial polyps.These three factors may serve as the reference indicators for the diagnosis of endometrial polyps.
