S100A8/A9在预测冠状动脉病变及其严重程度中的临床价值

【摘要】 目的分析S100A8/A9蛋白水平对预测冠状动脉(冠脉)病变及其严重程度中的临床价值。方法选取在2019年3月至2020年5月间就诊于中国医科大学附属盛京医院行冠脉造影术的患者225例,根据造影结果分为冠脉病变组(147例)和对照组(78例)。根据SYNTAX评分和Gensini评分评估患者冠脉病变的严重程度。使用SYNTAX评分将冠脉病变组分为轻度冠脉病变组(92例),中度冠脉病变组(33例)和重度冠脉病变组(22例)。使用Gensini评分将患者分为轻微冠脉病变组(89例)和严重冠脉病变组(58例)。收集患者血液样本检测S100A8/A9蛋白水平。结果存在冠脉病变患者的S100A8/A9水平明显高于健康人[0.86(0.37~1.47)vs. 0.30(0.22~0.45)μg/ml,P<0.001]。S100A8/A9蛋白是出现冠脉病变的独立危险因素[OR=18.58(5.29~65.18),P<0.001]。S100A8/A9浓度和SYNTAX评分具有正相关性(r=0.786,P<0.001)。S100A8/A9水平随病变严重程度加重逐渐升高。轻度病变与中、重度病变存在统计学差异(P<0.001),而中度病变与重度病变之间无显著差异(P=0.030)。S100A8/A9浓度和Gensini评分间具有正相关性(r=0.746,P<0.001)。根据ROC曲线结果得出血清S100A8/A9水平超过0.86μg/mL时认为患者可能存在严重冠脉病变的风险,此时灵敏度0.897,特异度0.775。结论 S100A8/A9水平在有冠脉病变的患者中显著升高,是其独立危险因素。S100A8/A9水平与冠脉病变严重程度相关,对严重冠脉病变有较好的预测价值。

【Abstract】 Objective To analyze the clinical value of S100 A8/A9 protein level in predicting coronary artery lesions(CAL) and their severity. Methods The patients(n=225) undergone coronary angiography(CAG) were chosen from Shengjing Hospital affiliated to China Medical University from Mar. 2019 to May 2020, and divided, according to CAG results, into CAL group(n=147) and control group(n=78). CAL severity was reviewed respectively according to SYNTAX scores and Gensini scores. According to SYNTAX scores, the patients were divided into mild CAL group(n=92), moderate CAL group(n=33) and severe CAL group(n=22). According to Gensini scores, the patients were divided into slight CAL group(n=89) and serious CAL group(n=58). The blood samples were collected for detecting S100 A8/A9 protein level. Results The level of S100 A8/A9 protein was significantly higher in CAL group than that in control group [0.86,(0.37~1.47) μg/ml vs. 0.30(0.22~0.45) μg/ml, P<0.001]. S100 A8/A9 protein was an independent risk factor of CAL [OR=18.58(5.29~65.18), P<0.001]. The level of S100 A8/A9 protein was positively correlated to SYNTAX scores(r=0.786, P<0.001). The level of S100 A8/A9 protein increased gradually along with CAL severity exacerbated. There was statistical difference among mild CAL group, moderate CAL group and severe CAL group(P<0.001), and there was no significant difference between moderate CAL group and severe CAL group(P=0.030). The level of S100 A8/A9 protein was positively correlated to Gensini scores(r=0.746, P<0.001). The results of ROC curve analysis showed that when S100 A8/A9 protein level was higher over 0.86 μg/ml, patients had higher CAL risks, and sensitivity was 0.897 and specificity was 0.775. Conclusion The level of S100 A8/A9 protein will increase significantly in CAL patients, which is an independent risk factor and closely correlated to CAL severity. The level of S100 A8/A9 protein has a higher predictive value to severe CAL.
