【摘要】 目的比较阿司匹林与氯吡格雷抗血小板治疗对急性脑梗死患者血小板聚集率的影响。方法回顾性分析2018年1月-2020年1月湖南省常德市第一人民医院收治的急性脑梗死患者120例的临床资料,根据患者抗血小板治疗药物不同分为2组,A组患者67例采用阿司匹林治疗,B组患者53例采用氯吡格雷治疗。治疗前和治疗7 d后采用PL-12血小板功能分析仪检测2组花生四烯酸(AA)诱导的血小板最大聚集率(MAR-AA)和二磷酸腺苷(ADP)诱导的血小板最大聚集率(MAR-ADP)。结果治疗7 d后,2组的MAR-AA、MAR-ADP均低于治疗前(P <0.05),2组MAR-AA比较差异无统计学意义(P> 0.05),B组MAR-ADP低于A组,差异有统计学意义(P <0.01)。2组的MAR-AA差值比较差异无统计学意义(P> 0.05),A组MAR-ADP差值中位数低于B组,差异有统计学意义(P <0.01)。结论阿司匹林与氯吡格雷抗血小板治疗急性脑梗死均可降低血小板最大聚集率,氯吡格雷较阿司匹林治疗更能抑制血小板聚集,抗血小板疗效更好。更多还原

【Abstract】 Objective To compare the effect of antiplatelet therapy of aspirin and clopidogrel on platelet aggregation in patients with acute cerebral infarction. Methods Clinical data of 120 patients with acute cerebral infarction admitted to the First People’s Hospital of Changde City,Hunan Province from January 2018 to January 2020 were retrospectively analyzed,according to antiplatelet therapy,the patients were divided into two groups,67 patients in group A were treated with aspirin and 53 patients in group B were treated with clopidogrel. Before treatment and 7 days after treatment,the platelet function analyzer PL-12 was used to detect the maximum platelet aggregation rate( MAR-AA) induced by arachidonic acid( AA) and the maximum platelet aggregation rate( MAR-ADP) induced by adenosine diphosphate( ADP) in the two groups. Results After7 days of treatment,the MAR-AA and MAR-ADP in both groups were lower than before treatment( P < 0. 05),there was no significant difference in MAR-AA between the two groups( P > 0. 05),while the MAR-ADP in group B was lower than that in group A,the difference was statistically significant( P < 0. 01). The MAR-AA difference between the two groups was not statistically significant( P > 0. 05),the median of MAR-ADP difference in group A was lower than that in group B,the difference was statistically significant( P < 0. 01). Conclusion Both aspirin and clopidogrel can reduce the maximum platelet aggregation rate in the treatment of acute cerebral infarction,clopidogrel can inhibit platelet aggregation more than aspirin,and the antiplatelet effect is better.更多还原
