视频脑电图在新生儿发作性事件监测中的诊断价值

【摘要】 目的探讨长程视频脑电图监测在新生儿发作性事件中的诊断价值。方法选取2018年6~12月在武汉儿童医院新生儿科住院治疗的有惊厥发作风险并进行过床旁长程视频脑电图监测的136例新生儿的临床资料,包括年龄、性别、诊断、脑电图背景活动、电临床发作及电发作,并电话随访6~12个月了解患儿的治疗及预后。结果136例患儿均通过脑电图有效监测,无1例发生意外,脑电信号清晰稳定。其中正常脑电图53例;轻、中、重度异常脑电图分别为49例、26例和8例。脑电图异常的严重程度与神经发育的不良预后明显相关(χ~2=13.8,P=0.003),癫痫性发作与神经发育的不良预后无明显相关性(χ~2=0.206,P=0.65),新生儿惊厥与继发性癫痫有明显相关性(χ~2=12.2,P=0.000)。结论利用长程视频脑电图监测能更好地提高对新生儿发作性事件的诊治能力,同时对预后有良好的评估作用。

【Abstract】 Objective To investigate the significance of long-term video electrophysiology(VEEG)monitoring in the prognosis of neonatal seizure.Methods The clinical data of V-EEG of 136 infants with neonatal convulsion risk from Wuhan Children’s Hospital between June 2018 and December2018 were prospective reviewed.Information collected included age of presentation,sex,diagnosis,EEG background,the frequency of clinical seizures and electrical seizures.During 6 to 12 months’ follow-up,the curative effects and prognosis were collected by telephone.Results The signals of EEG were clear and stable.There were no accidents during EEG moniroing through effective nursing.The normal EEG were seen in 53 newborns.The mild abnormal EEG,moderately abnormal EEG and remarkely abnormal EEG were seen in 49 newborns,26 newborns and 8 newborns respectively.The three abnormal levels of EEG were significantly related with neurological development(χ2=13.8,P=0.003).The correlation between seizures and prognosis had no statistical significance(χ2=0.206,P=0.65),while the correlation between seizures and secondary epilepsy had statistical significance(χ2=12.2,P=0.000).Conclusion Neonatal seizure was a common disease in the newborn,long-term VEEG monitoring could improve the diagnosis of it.
