【摘要】 目的:探讨脊柱侧弯患者美学期望、心理状态、社会支持程度与治疗依从性之间的关联。方法:选取2017年3月-2020年3月于笔者科室门诊诊治的187例接受支具治疗的脊柱侧弯患者作为研究对象,向患者发放依从性问卷、美学期望问卷、汉密顿抑郁量表(Hamilton depression scale,HAMD)、汉密顿焦虑量表(Hamilton anxiety scale,HAMA)、社会支持量表(Social support rating scale,SSRS)。随访半年后根据支具治疗依从性情况分为依从性差组和依从性好组,回收并统计量表评分后采用多元线性回归分析,美学期望、HAMD、HAMA、SSRS评分与依从性评分相关性采用Pearson相关性分析。结果:本次最终纳入174例患者(依从性好组102例,依从性差组72例)。依从性差组HAMD和HAMA评分高于依从性好组,依从性评分、美学期望值和SSRS评分低于依从性好组,差异均有统计学意义(P<0.05)。两组文化程度分布差异有统计学意义(P<0.05)。HAMD评分、HAMA评分、文化程度、美学期望值、SSRS评分为患者治疗依从性的影响因素;美学期望值、SSRS评分与依从性评分呈正相关(r=0.728、0.739,均P=0.000),HAMD、HAMA与依从性评分呈负相关(r=-0.685、-0.679,均P=0.000)。结论:脊柱侧弯患者较高的美学期望、良好的心理和社会因素关系着治疗意愿和配合程度,关注前述因素有助于提高患者治疗的依从性。更多还原

【Abstract】 Objective To explore association between aesthetic expectations,mental state,social support level and treatment compliance in scoliosis patients.Methods 187 scoliosis patients treated with orthosis treatment in our department from March 2017 to March 2020 were employed as research objects，adherence questionnaire,aesthetic expectations questionnaire,Hamilton depression scale(HAMD),Hamilton anxiety scale(HAMA),Social support rating scale(SSRS) were sent to patients.All patients were divided into the good compliance group and the bad compliance group according to orthosis treatment compliance after six months of follow-up.Univariate analysis and multiple linear regression were performed for related factors of orthosis treatment compliance after recycling and recording questionnaire,analysed association between aesthetic expectations,HAMD,HAMA,SSRS scores and compliance score by pearson correlation analysis.Results Finally,174 cases were employed (the good compliance group contained 102 cases,the bad compliance group contained 72 cases).HAMD and HAMA score in the bad compliance group were higher than those in the good compliance group,aesthetic expectation and SSRS score in the bad compliance group were lower than those in the good compliance group,the differences were statistically significant (P<0.05).There was statistical difference between two groups in the educational level (P<0.05).HAMD score,HAMA score,education level,aesthetic expectation and SSRS score were the influencing factors of treatment compliance.Aesthetic expectation and SSRS were positively correlated to compliance score(r=0.728,0.739,P=0.000),HAMD and HAMA were negatively correlated to compliance score(r=-0.685,-0.679,P=0.000).Conclusion Higher aesthetic expectation，good mental state and social factor are related to willingness and cooperation level for treatment in scoliosis patients，attaching importance to aforementioned factors will be beneficial to enhance patients’ treatment compliance.更多还原
