微输尿管镜治疗儿童输尿管下段结石11例疗效分析

【摘要】 目的探讨微输尿管镜在治疗儿童输尿管下段结石的安全性和有效性。方法 2016年6月至2020年5月十堰市太和医院泌尿外科收治儿童输尿管下段结石11例,均在全身麻醉下行微输尿管镜下钬激光碎石术,评估手术时间、住院时间及术后并发症发生情况,术后4周复查泌尿系CT评估结石清除率。结果所有患儿均成功完成微输尿管镜下钬激光碎石术,术中无患儿需行输尿管主动扩张或被动扩张,手术时间为(38.08±9.52)min;术后住院时间(5.12±0.58)d;术后1例患者出现发热,体温>38.5℃,为Clavien gradeⅡ并发症,按照药敏结果给予加强抗感染治疗后体温恢复正常;术后4周结石清除率为100%(11/11)。结论微输尿管镜可以安全、有效地应用于儿童输尿管下段结石的治疗,并发症低,能在造成损伤最小的情况下达到满意的结石清除效果。

【Abstract】 Objective To explore the safety and efficacy of micro-ureteroscopy in the treatment of lower ureteral calculi in pediatric patients.Methods A total of 11 children with lower ureteral calculi treated in our department during Jun.2016 and May 2020 were enrolled.All patients received micro-ureteroscopic holmium laser lithotripsy under general anesthesia.The operation time,hospitalization time and postoperative complications were evaluated.Urinary CT was performed 4 weeks after operation to evaluate the stone-free rate.Results All operations were successful,and no children needed active or passive ureteral dilation during operation.The average operation time was(38.08±9.52)min,and postoperative hospitalization was(5.12±0.58)d.Fever occurred in1 patient(T>38.5℃),which was a complication of Clavien gradeⅡ,and the temperature returned to normal after intensive anti-infection treatment.The stone-free rate after 4 weeks was 100%(11/11).Conclusion Micro-ureteroscopy is safe and effective in the treatment of lower ureteral calculi in pediatric patients.It causes few complications,and can achieve a satisfactory stone-free rate with minimal damage.
