超声心动图技术评价代谢综合征患者左心室功能损害及随访研究
Evaluation of the damage and follow-up study of left ventricular function in patients with metabolic syndrome by echocardiography
【摘要】 目的:应用超声心动图技术评价代谢综合征(MS)患者左心室功能,并对其进行3年随访,分析生活方式及药物干预对MS患者左心室功能的影响,探讨超声心动图技术在MS患者左心室功能评价及随访研究中的意义。方法:研究对象分为2组,病例组(MS组)90例,对照组40例,并进行3年随访,将MS组分为干预组、非干预组。获取左心室舒张功能参数(LAVI、E/A、e’、E/e,)与左心室收缩功能参数(LVEF、GLS、Bas-LS、Mid-LS、Api-LS、Bas-Rot、Api-Rot、Torsion)。结果 :与对照组相比,MS组E/A、e’、GLS、Bas-LS、Mid-LS、Api-LS减低(P<0.05),Api-Rot、Torsion增加(P<0.05)。多元线性回归分析显示SBP及HbA1c与GLS独立相关(β=-0.402,P<0.001;β=-0.253,P=0.008)。在随访后,与非干预组相比,干预组FPG增加,e’、GLS、Mid-LS、ApiLS减低(P<0.05),Torsion增加(P<0.05)。结论:MS患者左心室舒张与收缩功能均减低,其中收缩压和糖化血红蛋白是影响左心室收缩功能的主要因素;生活方式和药物干预可以延缓MS的左心室功能损害;超声心动图技术在MS患者左心室功能评价及随访研究中发挥着重要的临床作用。
【Abstract】 Objective: To evaluate the damage of left ventricular function and follow-up for 3 years in MS patients by echocardiography, and to analyze the effect of lifestyle and drug interventions on the left ventricular function of MS and explore the significance of echocardiography in the evaluation of left ventricular function and follow-up study in MS patients. Method:The subjects were divided into two groups, case group(MS group) included 90 patients, and control group included 40 healthy people. The patients were followed up for 3 years. The MS group was divided into intervention group and non-intervention group. Echocardiography was performed to acquire parameters of diastolic function of left ventricle(LAVI, E/A, e ’ and E/e’)and systolic function of left ventricle(LVEF, GLS, Bas-LS, Mid-LS, Api-LS, Bas-Rot, Api-Rot and Torsion). Results: Compared with control group, E/A, e’, GLS, Bas-LS, Mid-LS and Api-LS in MS group were significantly decreased(P<0.05), while Api-Rot and Torsion were significantly increased(P<0.05). Multiple linear regression analysis showed that SBP and HbA1 c were correlated independently with GLS(β=-0.402, P<0.001; β=-0.253, P=0.008). After follow-up, compared with non intervention group, FPG in intervention group was significantly increased(P<0.05). E’, GLS, Mid-LS, Api-LS in intervention group were significantly decreased(P<0.05) while Torsion was significantly increased(P<0.05). Conclusion: The left ventricular diastolic and systolic function are impaired in MS patients. SBP and HbA1 c may be the main factors effecting left ventricular systolic func-tion in MS patients. Lifestyle and drug intervention can delay the damage of left ventricular function in MS patients. Echocar-diography plays an important clinical role in the evaluation and follow-up study of left ventricular function in MS patients.
