老年高尿酸血症患者血尿酸水平与认知功能的相关性研究

【摘要】 目的研究老年高尿酸血症(HUA)患者血尿酸(SUA)水平与简易精神状态检查(MMSE)评分和认知功能异常之间的关系。方法 2019年10月—11月,以285例上海市宝山区大场镇社区常住居民为研究对象,检测其SUA水平、血压、血脂和MMSE评分等指标。将他们根据SUA水平按照三分位数分为低(T1)、中(T2)、高(T3) 3组,根据是否合并代谢综合征进行分层,分析SUA水平与MMSE评分以及认知功能异常的相关性。结果随着SUA水平升高,高密度脂蛋白胆固醇降低(P=0.010)、女性患者减少(P<0.001)。据代谢综合征分层,不同SUA水平组间的MMSE评分及认知功能异常差异无统计学意义。在HUA合并代谢综合征者中,T3组较T1组认知功能异常率增加(OR=2.89,95%CI:1.04~8.07,P=0.043)。在老年HUA患者及非合并代谢综合征人群中,SUA水平与MMSE评分和认知异常均无相关性。结论对于合并代谢综合征的老年HUA患者,在关注代谢类相关疾病治疗的同时,也应关注其认知功能的筛查及防治。

【Abstract】 Objective To study the relationship between serum uric acid(SUA) level and the Mini-Mental State Examination(MMSE) score and cognitive function abnormality in elderly patients with hyperuricemia(HUA).Methods From October to November 2019,285 permanent residents in Dachang Town,Baoshan District were recruited as the research subjects,and their SUA level,blood pressure,blood lipid,MMSE score and other indexes were tested.According to their SUA level,they were divide into three groups:low(T1),medium(T2),and high(T3).According to whether they suffered from metabolic syndrome,they were stratified,and the correlation between SUA level and MMSE score and cognitive function abnormality was analyzed.Results As the level of SUA increased,high-density lipoprotein-cholesterol decreased(P=0.010),and female patients decreased(P<0.001).According to the metabolic syndrome stratification,there was no statistically significant difference in MMSE scores and cognitive function abnormalities among groups with different SUA levels.In HUA patients with metabolic syndrome,the abnormal rate of cognitive function in T3 group was higher than that in T1 group(OR-2.89,95%CI:1.04-8.07,P=0.043).In elderly HUA patients and people without metabolic syndrome,SUA levels were not correlated with MMSE scores and cognitive function abnormality.Conclusion For the elderly patients with hyperuricemia and metabolic syndrome,we should pay attention to both the treatment of metabolic-related diseases and the screening and prevention of cognitive function abnormality.
