低度近视性屈光参差儿童主导眼调节参数研究
Accommodative function of the dominant eye in children with low myopic anisometropia
【摘要】 目的研究低度近视性屈光参差儿童的主导眼与非主导眼的屈光度、调节幅度、调节灵活度、正负相对调节、调节反应的差异。方法回顾性临床研究。收集眼科门诊27例能配合检查的低度近视性屈光参差儿童(1.0 D≤双眼等效球镜差<2.5 D),矫正视力≥0.8。采用"卡洞法"测量被检者的主导眼,分别记录主导眼和非主导眼的屈光度,然后测量各调节参数。用移近+负镜法测量调节幅度,用正负球镜翻转法测量调节灵活度,用+0.25 D球镜逐渐增加测量负相对调节,用-0.25 D球镜逐渐增加测量正相对调节,用融合交叉柱镜法测量调节反应,比较其主导眼与非主导眼的差别。结果低度近视性屈光参差患儿主导眼的等效球镜屈光度高于非主导眼,主导眼与屈光度较高的眼一致率高于非主导眼;主导眼调节滞后量显著高于非主导眼(P <0.001),正相对调节低于非主导眼,差异有统计学意义(P <0.001),主导眼的调节幅度、调节灵活度、负相对调节与非主导眼的差异无统计学意义(P> 0.05)。结论低度近视性屈光参差儿童的主导眼更倾向于是近视较高度眼,主导眼有较非主导眼明显的调节滞后,正相对调节数值主导眼低于非主导眼。
【Abstract】 Objective To investigate the accommodative difference between the dominant and non-dominant eye in children with low myopic anisometropia. Methods A total of 27 children with low myopic anisometropia( defined as binocular equivalent spherical aberration less than 2. 5 D and greater than or equal to 1. 0 D),and whose best corrected visual acuity were not worse than 0. 8 were retrospectively analyzed. Dominant eyes were determined with the hole-in-the-card test,and the refractive error of each eye was recorded. The accommodative amplitude( AA) was measured with improved pushup test,the accommodative facility( AF) was measured by turning over convex and concave lens test,the accommodative response( AR) was measured with fusion crossed-cylinder,and the positive/negative relative accommodation( PRA/NRA)was measured by ± 0. 25 D spherical progressive increase. The measurements between the dominant eye and non-dominant eye were compared. Results Among low myopic anisometropia children,the mean spherical equivalent of the dominant eye was higher than the non-dominant eye,with more myopic refractive error was observed in the dominant eyes( P < 0. 05).The AR in the dominant eye was significantly higher than the non-dominant eye,while the PRA was lower,and the difference was statistically significant( P < 0. 001). There was no significant difference in AA,AF or NRA between the dominant and non-dominant eyes( P > 0. 05). Conclusions The dominant eye of low myopic anisometropia children is more likely to have more myopic refractive error. Accommodative lag in dominant eye is higher than non-dominant eye,while PRA is lower.
