脑裂头蚴病的临床特征及外科手术治疗分析

【摘要】 目的总结脑裂头蚴病的外科手术治疗病例的临床特点和手术经验。方法回顾性分析自2011年1月至2020年12月在复旦大学附属华山医院神经外科接受手术治疗的19例脑裂头蚴病患者的临床资料。结果 19例患者均接受神经导航下开颅手术治疗,其中1例患者接受了2次手术,2例患者先行导航下穿刺活检后改开颅手术治疗。19例病例中,取出活虫体14例,取出死的完整虫体1例,未取出虫体4例。随访6个月至9.4年,失访1例。术后复查MRI增强,13例强化病灶消失。16例术前有癫■患者,随访时癫■达到改良Engel分级I级者占62.5%(10/16例)。术后未见明显出血、梗死、肢体活动障碍等并发症。结论外科手术治疗脑裂头蚴病安全有效。

【Abstract】 Aim To summarize the clinical characteristics and surgical experience of cerebral sparganosis. Methods Nineteen cerebral sparganosis patients who received surgical treatment at our center(Department of Neurosurgery, Huashan Hospital, Fudan University) between January 2011 and December 2020 were enrolled in this study. The demographic information, clinical data and the treatment were retrospectively collected. Results All 19 patients underwent MR-guided craniotomy. Among them, one patient received twice operations. Two patients underwent MR-guided aspiration and turned to craniotomy during the operation. Live larvae were identified in 14 patients, dead larvae were identified in one patient,only eosinophilic tunnels were identified in 4 patients. Follow-up was 6 months to 9.4 years with one lost to follow-up. Enhanced lesions disappeared in 13 cases after surgery. Ten out of 16(62.5%) patients who had preoperative epilepsy, got a modified Engel grade I after the operation. There were no obvious complications and sequelae after the operation. Conclusion Surgical removal of cerebral sparganosis is effective and safe.
