抗阻训练对改善帕金森病运动症状疗效的Meta分析
Meta Analysis of Effect of Resistance Training on Motor Symptoms of Parkinson’s Disease
【摘要】 目的评价抗阻训练对帕金森病(PD)运动相关症状的疗效。方法对PubMed、Embase和Web of Science进行全面系统的数据库检索,然后对研究项目进行评估,以确定是否有可纳入研究分析。对纳入的研究项目进行了质量评价、偏倚分析和敏感性分析,当存在较大异质性时分析异质性来源,必要时进行亚组分析。选择冻结步态、肌肉力量、平衡、运动协调能力和生活质量相关结果作为主要观察指标。结果共纳入11篇随机对照试验文献并进行了分析。抗阻训练对PD患者肌力(SMD=0.85,95%CI:0.55~1.16,P<0.000 01)、UPDRS运动部分(MD=-2.31,95%CI:-3.67~-0.94,P=0.001)、快速步行速度(MD=0.26,95%CI:0.17~0.34,P<0.000 01)和生活质量(MD=-5.98,95%CI:-8.55~-3.41,P<0.000 01)的改善差异有统计学意义。PD患者中,除运动障碍-僵硬亚型外,冻结步态明显改善(P=0.02)。平衡能力改善,但差异无统计学意义(P=0.99)。结论抗阻训练能改善PD患者的肌肉力量、UPDRS运动部分评分、快速步行速度和生活质量,是PD的有效辅助治疗方法。
【Abstract】 Aim To evaluate the effect of resistance training on motor related symptoms of Parkinson’s disease(PD).Methods PubMed,Embase and Web of Science databases were searched comprehensively and systematically,and then the study was evaluated to determine whether it is possible to be included in the study.The quality evaluation,bias analysis and sensitivity analysis were carried out for the included studies.When there was large heterogeneity,the sources of heterogeneity were analyzed,and subgroup analysis was conducted when necessary.Freezing of gait,muscle strength,balance,motor coordination and quality of life were selected as the main observation indexes.Results A total of 11 randomized controlled trials were included and analyzed.Resistance training improved muscle strength(SMD=0.85,95%CI:0.55-1.16,P<0.000 01),motor part of UPDRS(MD=-2.31,95%CI:-3.67--0.94,P=0.001),fast walking speed(MD=0.26,95%CI:0.17-0.34,P<0.000 01) and quality of life(MD=-5.98,95%CI:-8.55--3.41,P<0.000 01) in PD patients.In patients with PD,except for the akinesia and rigidity subtype,freezing gait was significantly improved(P=0.02).The improvement of balance ability was not statistically significant(P=0.99).Conclusion Resistance training can improve the muscle strength,UPDRS motor part score,fast walking speed and quality of life,and is an effective adjuvant treatment for Parkinson’s disease.
