维持性血液透析中低血压诊治的研究进展

【摘要】 维持性血液透析(maintenance hemodialysis, MHD)治疗是终末期肾脏病患者一种重要的肾脏替代治疗措施。然而,当透析过程中液体的去除速度过快,超过血浆补充的速度以及相关的心血管、神经、激素等代偿反应的速度时,就会发生透析中低血压(intradialytic hypotension, IDH),其可造成全身多处器官、系统的损害,甚至危及MHD患者生命。但随着对IDH发生机制研究的不断深入,近年来低温透析、生物反馈透析、间歇补充型血液透析滤过等措施的出现,降低了IDH的发生,改善了MHD患者的预后。本文将近年来国内外学者对IDH相关的研究进行探讨和概述,为IDH防治的进一步研究提供参考。

【Abstract】 Maintenance hemodialysis(MHD)is an important renal replacement therapy for patients with end-stage renal disease.However, when body fluid is removed too fast during dialysis as to exceed the rate of plasma supplement and the related cardiovascular, nervous, hormonal and other compensatory reactions, intradialytic hypotension(IDH)causes damage to multiple organs and systems and even endangers the lives of MHD patients.However, with a deepening mechanistic understanding of IDH,its occurrence of IDH has declined and the prognosis of MHD patients has been improved with the emergence of hypothermic dialysis, biofeedback system and intermittent infusion hemodiafiltration in recent years.Here both domestic and foreign IDH-related researches were summarized to provide references for further studies on the prevention and treatment of IDH.
