二硅酸锂基玻璃陶瓷高嵌体修复下颌第一磨牙抗折强度的体外研究

【摘要】 目的:观察计算机辅助设计/计算机辅助制造(computer aided design/computer aided manufacture, CAD/CAM)二硅酸锂基玻璃陶瓷高嵌体与全冠修复对根管治疗后患者邻面缺损下颌第一磨牙抗折强度的影响。方法:选取60颗离体下颌第一磨牙分为A组(n=34)与B组(n=26),A组根管治疗后使用CAD/CAM二硅酸锂基玻璃陶瓷高嵌体修复,B组根管治疗后使用全冠修复。比较两组试件折裂载荷值、破坏模式。结果:两组无机械循环+冷热循环试件折裂载荷值均较机械循环+冷热循环高(P<0.05),A组机械循环+冷热循环试件折裂载荷值明显高于B组机械循环+冷热循环试件(P<0.05)。A组Ⅲ类、Ⅳ类破坏模式占比明显高于B组(P<0.05)。结论:邻面缺损下颌第一磨牙进行根管治疗后,CAD/CAM二硅酸锂基玻璃陶瓷高嵌体以及全冠修复方式均表现出较高即刻抗折强度,以Ⅲ类与Ⅳ类破坏模式为主。

【Abstract】 Objective:To observe the effects of computer aided design/computer aided manufacture(CAD/CAM)lithium disilicate glass ceramic inlays and full crown restoration on fracture resistance of mandibular first molar after root canal therapy for interproximal defect.Methods:Sixty extracted mandibular first molars were selected and divided into group A(n=34)and group B(n=26)according to the treatment method.Group A was treated with CAD/CAM lithium disilicate glass ceramic inlays after root canal therapy, and group B was treated with full crown restoration after root canal therapy.The fracture load value and failure mode were compared between the two groups.Results:The fracture load values of non-mechanical cycle+thermocycling in both groups were higher than mechanical cycle+thermocycling(P<0.05).The fracture load values of mechanical cycle+thermocycling in group A were significantly higher than those in group B(P<0.05).The proportions of Class III and IV failure modes in group A were significantly higher than group B(P<0.05).Conclusion:After root canal therapy for interproximal defect, both CAD/CAM lithium disilicate glass ceramic inlays and full crown restoration show high immediate fracture resistance, and fracture resistance of inlays is higher at 5 years after restoration.The failure modes mainly include Class III and IV.
