【摘要】 兴奋激越是精神科常见急症,伴有明显的冲动攻击等风险,需要快速有效处置。齐拉西酮注射液是我国首个第二代抗精神病药速效注射剂型,用于治疗成人精神分裂症患者的急性激越症状。临床实践中,医生会选择齐拉西酮注射液治疗其他精神障碍患者的兴奋激越,属于超说明书用药。为了科学指导齐拉西酮注射液超说明书用药,中华医学会精神医学分会精神分裂症协作组组织专家,系统汇总了齐拉西酮注射液超说明书用药的相关研究文献,并依据定量系统评价证据分级工具(GRADE)系统的循证医学证据与推荐等级标准,对齐拉西酮注射液超说明书使用进行了不同等级的推荐,警示了需要特别关注的潜在风险,并提出了相应的处置方法。更多还原

【Abstract】 Objective: Acute agitation is a common psychiatric emergency with high risk of impulsive and aggressive behaviors, in which an efficient intervention should be urgently provided in daily clinical practice.Ziprasidone, the first rapid release intramuscular formulation of second-generation antipsychotics available in China, is indicated only for the treatment of acute agitation in adult patients with schizophrenia.However, in the clinical practice, clinicians also actually prescribe intramuscular ziprasidone for the agitation treatment in patients with mental disorders other than schizophrenia, which are considered as off-label use.In order to guide the off-label use of intramuscular ziprasidone, a group of experts were invited by the Schizophrenia Collaborative Group of the Chinese Psychiatry Society to systematically review the literatures on off-lable use of injectable ziprasidone.Based on the Grading of Recommendations Assessment, Development and Evaluation(GRADE)system some items of consensus are reached on levels of the strength of evidences and recommendations for the off-lable use of intramuscular ziprasidone.And warnings for the potential risks and related interventions were also addressed.更多还原
