预弯钢板+髂腹股沟入路微创经皮钢板内固定技术治疗骨盆前环骨折
Treatment of anterior pelvic ring fracture with pre-bending plate and minimally invasive percutaneous plate osteosynthesis technique
【摘要】 目的探讨预弯钢板+髂腹股沟入路微创经皮钢板内固定技术(MIPPO)治疗骨盆前环骨折的疗效。方法将50例骨盆前环骨折患者根据手术方式不同分为传统手术组(采用髂腹股沟入路传统手术治疗,25例)及微创手术组(采用预弯钢板+髂腹股沟入路MIPPO治疗,25例)。比较两组手术情况及临床疗效。结果手术时间、术中出血量、切口长度微创手术组短(少)于传统手术组(P <0.001)。患者均获得随访,时间12~48个月。两组均在术后4~6周出现骨痂,术后3个月骨折达到临床愈合标准。术后并发症发生率微创手术组低于传统手术组(P <0.05)。末次随访时Majeed评分优良率两组比较差异无统计学意义(P> 0.05)。结论预弯钢板+髂腹股沟入路MIPPO治疗骨盆前环骨折疗效确切,可明显缩短手术时间,减少术中出血量和术后并发症的发生。
【Abstract】 Objective To investigate the effects of pre-bending plate and minimally invasive percutaneous plate osteosynthesis( MIPPO) in the treatment of anterior pelvic ring fracture. Methods The 50 patients with anterior pelvic ring fractures were divided into two groups according to the different operation mothods,the 25 patients in the conventional group were performed with conventional surgery via ilioinguinal approach,and orther 25 patients in the MIPPO group were treated by pre-bending plate plus MIPPO with ilioinguinal approach. The operation situation and clinical effect were compared between two groups. Results The operative time,intraoperative bleeding volume and incision length in the MIPPO group were shorter( less) than those of the conventional group( P < 0. 001). All patients were followed up for 12 ~ 48 months. The bone callus formation was within postoperative 4 ~ 6 weeks in two groups,the fracture reached clinical healing criterion within 3 months. The postoperative complication incidence rate in the MIPPO group was lower than that of the conventional group( P < 0. 05). At the last follow-up,the excellent-good rate of Majeed score had no difference between two groups( P > 0. 05). Conclusions Compared with the conventional approach,pre-bending plate plus MIPPO are used in the treatment of anterior pelvic ring fracture via the ilioinguinal approach,which can significantly shorten the operation time,reduce intraoperative bleeding volume and postoperative complications.
