两点一线精准穿刺经皮椎体成形术治疗骨质疏松性椎体压缩骨折

【摘要】 目的探讨两点一线精准穿刺经皮椎体成形术治疗骨质疏松性椎体压缩骨折(OVCF)的临床疗效。方法将62例OVCF患者按入院时间不同分为对照组(采用经皮椎体成形术治疗,31例)和观察组(采用两点一线精准穿刺经皮椎体成形术治疗,31例)。记录术中穿刺次数、术中透视次数、手术时间、骨水泥渗漏率、术后3 d疼痛VAS评分。结果两组术后均未出现脊髓神经损伤、感染、骨水泥中毒反应及渗漏引起的神经压迫症状等并发症。术中穿刺次数、术中透视次数、手术时间观察组明显少(短)于对照组,差异均有统计学意义(P <0.01)。术后3 d VAS评分、骨水泥渗漏率两组比较差异均无统计学意义(P> 0.05)。结论采用两点一线精准穿刺经皮椎体成形术治疗OVCF在术中穿刺次数、术中透视次数、手术时间方面具有明显优势。

【Abstract】 Objective To investigate the clinical effect of two-point and one-line accurate puncture in percutaneous vertebroplasty( PVP) in the treatment of osteoporotic vertebral compression fracture( OVCF). Methods The 62 patients with OVCF were divided into control group( 31 cases were treated with PVP) and observation group( 31 cases were treated with two-point and one-line accurate puncture PVP). The intraoperative puncture times,X-ray fluoroscopy times,operation time,bone cement leakage rate and pain VAS at 3 d postoperation were recorded. Results There were no symptoms of spinal cord injury,infection,bone cement toxic reaction and nerve compression caused by bone cement leakage in both groups. The intraoperative puncture times,X-ray fluoroscopy times and operation time of the observation group were significantly less( shorter) than those of the control group,and the differences were statistically significant( P < 0. 01). There were no significant differences in VAS at 3 d after operation and bone cement leakage rate between the two groups( P > 0. 05). Conclusions Two-point and one-line accurate puncture in PVP for OVCF have obvious advantages in intraoperative puncture times,X-ray fluoroscopy times and operation time.
