三维斑点追踪技术评价2型糖尿病合并高尿酸血症患者左室整体收缩功能

【摘要】 目的探讨三维斑点追踪(3D-STI)技术评价2型糖尿病(T2DM)合并高尿酸血症(HU)患者左室整体收缩功能的临床价值。方法选取62例T2DM患者,根据是否合并HU分为单纯T2DM组33例和T2DM+HU组29例,另选健康体检者40例为对照组,各组均行常规和三维超声心动图检测,应用3D-STI分析软件获取并比较各组间3D-STI参数的差异,分析其影响因素。结果单纯T2DM组左室整体纵向应变(GLS)低于对照组,差异有统计学意义(P<0.05),左室整体圆周应变(GCS)、面积应变(GAS)及径向应变(GRS)与对照组比较差异均无统计学意义。T2DM+HU组GLS、GAS、GCS及GRS均低于对照组和单纯T2DM组,差异均有统计学意义(均P<0.05)。空腹血糖和血尿酸是左室各方向整体应变值的独立影响因素。结论 3D-STI可以早期敏感地检出T2DM合并HU患者左室整体收缩功能受损情况,为临床提供可靠依据。

【Abstract】 ObjectiveTo analyze the left ventricular global systolic function with three-dimensional speckle tracking imaging(3 D-STI)in patients with type 2 diabetes mellitus(T2 DM)complicated with hyperuricemia(HU).MethodsSixty-two patients with T2 DM were divided into T2 DM group(33 cases)and T2 DM+HU group(29 cases)according to whether they had HU,and 40 healthy subjects were selected as the control group.All groups were examined by routine and three-dimensional echocardiography.The 3 D-STI analysis software was used to obtain and compare the differences in 3 D-STI parameters between the groups,and the influencing factors were analyzed.ResultsLeft ventricular global longitudinal strain(GLS)in T2 DM group was lower than that in control group,and the difference was statistically significant(P<0.05).There were no significant difference in left ventricular global circumscribed strain(GCS),area strain(GAS),and radial strain(GRS)between the T2 DM group and control group.GLS,GAS,GCS and GRS in T2 DM+HU group were lower than those in control group and T2 DM group,and the differences were statistically significant(all P<0.05).Fasting blood glucose and serum uric acid were independent influencing factors of left ventricular global strain values in all directions.Conclusion3 D-STI can sensitively detect the impaired left ventricular global systolic function in patients with T2 DM complicated with HU at early stage,providing a reliable basis for clinical practice.
