【摘要】 目的:探讨日间手术模式在透析用血管通路手术中的运行流程及应用特点。方法:回顾性分析南京医科大学第二附属医院从2017年9月1日—2018年3月31日152例接受血管通路日间手术患者的临床特点以及血管通路日间手术流程、安全性及有效性。结果:152例患者中平均年龄56岁且54.3%为男性,自体动静脉内瘘(arteriovenous fistula, AVF)成形术、B超引导下经皮腔内血管成形术(percutaneous translumin alangioplasty, PTA)、切开取栓联合PTA杂交手术以及带隧道和涤纶套的透析导管(tunnel-cuffed cather, TCC)置入术分别占31.6%、53.9%、9.2%、5.3%。手术成功率为100%,无并发症的发生,患者平均手术时间为(68.3±10.6)min,日间病房平均住院时间为(10.3±5.8) h,平均住院费用为(7 749.2±3 013.1)元。随访30 d初级通畅率为100%。结论:日间手术模式优化了患者的就医模式,显著提高了血管通路手术的效率,更大化地利用了有限的医疗资源。更多还原

【Abstract】 Objective:To explore the clinical efficacy and characteristic of ambulatory surgery for vascular acces construction. Methods:The clinical data of 152 patients who underwent ambulatory surgery to construct vascular access at the Second Affiliated Hospital of Nanjing Medical University from September 2017 to March 2018, and the procedures、safety and efficacy of day surgery weren retrospectively analyszed. Results:The mean age was56 years and 54.3% were male. With regard to surgery type, 31.6%、53.9%、9.2%、5.3% of vasculare access were treated with arteriovenous fistula、percutaneous translumin alangioplasty、hybrid surgery and tunnel-cuffed cather respectively. The operation success rate was 100% without complications. The average operation was(68.3±10.6)min; the average hospital stay in the day ward was(10.3±5.8)h; the average hospitalization cost was(7 749.2±3 013.1)yuan. The primary patency rate was 100% at 30 days. Conclusion:Ambulatory surgery optimizes the medical treatment model, improves the efficiency of vascular access surgery significantly, and makes greater use of limited medical resources.更多还原
