【摘要】 目的 探讨骨髓细胞形态学检查的初步报告在初诊血液肿瘤患者早期诊断和治疗中的临床价值。方法 建立骨髓细胞形态学初步诊断积分系统（The preliminary morphological diagnosis scoring system, PMDSS）及评判标准，对达标者进行形态学初步报告。将患者的初步报告和正式报告分别与确诊报告进行临床符合度（Clinical coincidence score, CCS）分析。CCS满分为4分，≥3分表示两者相符。结果 收集到848名患者的数据，符合PMDSS报告标准有820例（96.70%）。完成初步报告者有679例（80.07%），其CCS得分为(3.53±0.70)分；CCS≥3分者初步报告640例（94.26%）、正式报告660例（97.20%），两者差异无统计学意义P>0.05。结论 骨髓细胞形态学初步报告对初诊血液肿瘤的早期诊疗方向具有重要价值，是实验室主动参与临床急救血液肿瘤患者的重要方法。更多还原

【Abstract】 Objective To explore the clinical value of the preliminary report of marrow cytomorphology in the early diagnosis and treatment of newly diagnosed patients with hematological malignancies. Methods The preliminary morphological diagnosis scoring system(PMDSS) and its evaluation criteria were established. This system was used to make a preliminary morphological report on those patients who meet the criterion. The preliminary report and the formal report of the patient were compared with the confirmed report by the clinical coincidence score(CCS) analysis, respectively. The maximum score of CCS is 4 points, and ≥3points means that both reports are consistent. Results The data from 848 patients were collected and 820(96.70%) met the reporting criterion of PMDSS. There were 679 patients(80.07%) who completed the preliminary report, and their CCS score was 3.53±0.70. Among these 679 patients, the preliminary report with CCS≥3 was 640 cases(94.26%) and the formal report with CCS≥3 was 660(97.20%). There was no statistical difference between the preliminary report and the formal report(P>0.05). Conclusion The preliminary report of marrow cytomorphology plays a key role in the early diagnosis and treatment of newly diagnosed patients with hematological malignancies, and is an important method for laboratories to participate in clinical emergency treatment of these patients.更多还原
