【摘要】 目的比较高孕激素状态下促排卵(PPOS)方案与其他3种不同促排卵方案的实验室和临床妊娠结局。方法回顾性分析2019年6月～2020年1月在我院生殖中心行体外受精/卵泡浆内单精子显微注射(IVF/ICSI)共430个取卵周期和256个冻融周期,观察实验室相关数据、受精情况及妊娠结局的差异。结果取卵周期中,PPOS方案82个,长方案152个,短方案116个,拮抗剂方案80个。PPOS组获卵数(11.1±7.3)、受精率(85.6%)、卵裂率(95.1%)、可用胚胎数(4.5±3.1)、优质胚胎率(20.2%)等与其他3组之间差异均无统计学意义(P>0.05)。冻融周期中,PPOS方案27个周期,拮抗剂方案107个,长方案78个,短方案44个。PPOS组种植率(26.2%)、临床妊娠率(63.0%)和流产率(11.8%)与其他各组比较差异无显著性(P>0.05),但临床妊娠率和种植率高于其他各组(P>0.05)。结论 PPOS方案和其他3种促排卵相比,对卵泡发育、卵母细胞和胚胎质量没有影响,而且临床妊娠率和种植率有升高的趋势,可以作为常规促排卵方案的一种新的选择。更多还原

【Abstract】 Objective To analyze the clinical outcomes of progestin primed ovarian stimulation (PPOS compared with the other three different controlled ovarian hyperstimulation (COH) protocols in fresh embryo transfer (ET)and frozen-thawed embryo transfer (FET) cycles. Methods A total of 430 oocyte pick-up cycles and 272 FET cycles were retrospectively analyzed. Number of oocytes retrieved,laboratory indexes and pregnancy outcome of FET were compared.Results The mean oocytes retrieved (11. 1 ± 7. 3),fertilization rate (85. 6%),cleavage rate (95. 1%) and excellent embryo rate (20. 2%) as well as transplantable embryo rate (4. 5 ± 3. 1) of the PPOS group did not show significant differences compared with the other 3 subgroups (all P<0. 05) in fresh cycle. As for pregnancy outcomes in FET cycles,no statistically significant differences were observed among the four groups in embryo implantation rate (26. 2%),clinical pregnancy rate (63. 0%) and abortion rate (11. 8%) (all P < 0. 05). However,embryo implantation rate,clinical pregnancy rate was higher in PPOS group compared with the other groups. Conclusion Compared with the other three ovulation stimulation programme,PPOS might be used as a new alternative for controlled ovulation stimulation protocols.更多还原
