【摘要】 目的:比较使用自体移植物、混合移植物和同种异体移植物重建前交叉韧带后的临床和二次关节镜检查结果。方法:选择行ACL重建并接受二次关节镜检查的患者,通过Lysholm评分、国际膝关节文件委员会(IKDC)评分、Tegner活动度评分以及KT-1000测量的侧向差异来比较临床结果。结果:三组间Lysholm评分、IKDC评分、Tegner活动度评分差异均无统计学意义(P>0.05)。KT-1000检查显示同种异体移植物较自体移植物或混合移植物韧带更易松弛。自体骨移植组滑膜覆盖率明显优于杂交骨移植组和同种异体骨移植组,而杂交骨移植组和同种异体骨移植组滑膜覆盖率差异无统计学意义(P>0.05)。在移植物张力方面,自体组和杂交组相似,但优于同种异体组。结论:自体和混合肌腱移植物重建前交叉韧带的疗效与同种异体肌腱移植物相同,但膝关节稳定性好于同种异体移植物。此外,自体肌腱移植物比其他两种类型的移植物显示出更好的滑膜覆盖率。更多还原

【Abstract】 Objective To compare the clinical and secondary arthroscopic results of anterior cruciate ligament reconstruction with autografts, mixed grafts and allografts.Method The patients who underwent ACL reconstruction and underwent secondary arthroscopy in our hospital.The clinical results were compared by Lysholm score, International knee documentation Committee(IKDC)score, Tegner range of motion score and lateral difference of KT-1000 measurement.Results There was no significant difference in Lysholm score, IKDC score and Tegner activity score among the three groups(P>0.05).KT-1000 examination showed that the ligaments of allografts were more relaxed than those of autografts or mixed grafts.The synovial coverage rate of autogenous bone transplantation group was significantly better than that of hybrid bone transplantation group and allogeneic bone transplantation group, but there was no significant difference between hybrid bone transplantation group and allogeneic bone transplantation group(P>0.05).In terms of graft tension, the autologous group was similar to the hybrid group, but better than the allogeneic group.Conclusion The curative effect of anterior cruciate ligament reconstruction with autogenous and mixed tendon graft is the same as that of allogeneic tendon graft, but the stability of knee joint is better than that of allogeneic tendon graft.In addition, autologous tendon grafts showed better synovial coverage than the other two types of grafts.更多还原
