常见外阴阴道炎与高级别宫颈上皮内病变的相关性研究

【摘要】 目的:了解常见外阴阴道炎与高级别宫颈上皮病变(CIN)的相关性。方法:选取2015年1月~2018年12月收治的237例高级别宫颈上皮内病变患者作为试验组,与同期随机选取的200例非宫颈病变患者作为对照组,回顾分析两组阴道分泌物pH值,清洁度和细菌性阴道病、外阴阴道假丝酵母菌病和滴虫性阴道炎发病情况。结果:试验组阴道分泌物pH值平均值高于对照组,分别为(5.48±0.33)和(5.16±0.33),清洁度Ⅲ度~Ⅳ度试验组明显高于对照组(比率分别73.83%和28.50%);试验组细菌性阴道病比率14.346%,明显高于对照组5.000%,但回归分析后差异无统计学意义(P>0.05)。而外阴阴道假丝酵母菌病和滴虫性阴道炎发病情况相比较,差异无统计学意义(P>0.05)。结论:外阴阴道炎的阴道微环境改变促进CIN的发展,重视常见外阴阴道炎的治疗,维护阴道微环境对于预防CIN具有一定意义。

【Abstract】 Objective To study the relationship between vulvovaginitis and high grade cervical intraepithelial neoplasm(CIN).Method 237 patients who were diagnosed as high grade CIN from January 2015 to December 2018 were admitted to test group; and 200 patients whose cervical screen were normal during the same period were selected at random as control group.The following items were analyzed: vaginal pH,cleanliness, and the incidence rate of bacterial vaginosis, vulvovagilnal candidiasis and trichomonal vaginitis.Results The vaginal pH were(5.48±0.33) in test gruop, higher than that(5.16±0.33) in control group.The cleanliness of Ⅲ~Ⅳ in test group was 73.83%,higher than that(28.50%)in control group.There was significant difference in the morbidity of bacterial vaginosis in test group(14.346%)compared with that(5.000%)in control group.There were no difference in the incidence rate of vuvovaginal candidiasis and trichomonal vaginitis between the two groups(P>0.05).Conclusion The change of vaginal microenviroment may contribute to development of CIN,and we should pay attention to therapy to the vulvovaginitis and keep the balance of the vaginal microenviroment to prevent CIN.
