【摘要】 目的对比全膝关节置换术与单髁关节置换术在单间室骨关节炎中的治疗效果。方法方便选择2018年5月—2020年4月该院收治的90例单间室骨关节炎患者,以随机数字表法将其分为两组,其中对照组45例行全膝关节置换术治疗,研究组45例行单髁关节置换术治疗。对比两组患者的手术指标、治疗效果、膝关节自主屈曲至90°的时间、术后1 d时疼痛视觉模拟量表(VAS)评分与住院时间。结果研究组手术时间(58.2±5.0)min、术中出血量(135.3±20.3)mL、手术切口(7.0±1.5)cm低于对照组(78.2±5.6)min、(245.6±25.3)mL、(12.0±2.0)cm,差异有统计学意义(t=17.871、22.811、13.416,P<0.001)。研究组治疗的优良率为91.11%与对照组88.89%对比差异无统计学意义(χ~2=0.124,P>0.05)。研究组膝关节自主屈曲至90°的时间(6.5±3.6)d、术后1 d时VAS评分(2.6±1.2)分与住院时间(7.0±1.2)d均低于对照组(9.5±4.0)d、(3.3±1.0)分、(10.2±1.0)d,差异有统计学意义(t=3.740,3.006,13.742,P<0.05)。结论全膝关节置换术与单髁关节置换术在单间室骨关节炎中的治疗效果相当,但单髁关节置换术创伤性更小,疼痛更轻微,进一步加快了患者的康复速度。更多还原

【Abstract】 Objective To compare the therapeutic effects of total knee arthroplasty and unicompartmental arthroplasty in single-compartment osteoarthritis. Methods Convenient select Ninety patients with single-compartment osteoarthritis admitted to the hospital from May 2018 to April 2020 were selected and divided into two groups by random number table method. Among them, 45 patients in the control group underwent total knee arthroplasty. The study group of 45 patients underwent unicondylar joint replacement. The surgical indicators, treatment effects, time for knee joint flexion to 90°, pain visual analog scale(VAS) score and hospital stay were compared between the two groups of patients. Results The operation time of the study group was(58.2±5.0) min, the intraoperative blood loss(135.3±20.3) mL, and the surgical incision(7.0±1.5) cm were lower than those in the control group(78.2±5.6) min,(245.6±25.3) mL,( 12.0±2.0) cm,the difference was statistically significant(t=17.871, 22.811, 13.416, P<0.001). The excellent and good rate of treatment in the study group was91.11% compared with 88.89% in the control group, and there was no statistically significant difference(χ~2=0.124, P>0.05).The time of knee joint flexion to 90° in the study group(6.5±3.6) d, VAS score(2.6±1.2)points and hospitalization time(7.0±1.2) d at 1 day after operation were lower than those in the control group(9.5±4.0) d,(3.3±1.0) points,(10.2±1.0)d,the difference was statistically significant(t =3.740, 3.006, 13.742, P <0.05). Conclusion Total knee arthroplasty and unicompartmental arthroplasty have similar therapeutic effects in single-compartment osteoarthritis, but unicompartmental arthroplasty is less traumatic and less painful, which further accelerates the patient’s recovery speed.更多还原
