分析心脏彩超在冠心病患者节段性室壁运动异常诊断中的临床研究
Analysis on the Clinical Research of Cardiac Color Doppler Ultrasound in the Diagnosis of Segmental Ventricular Wall Motion Abnormalities in Patients with Coronary Heart Disease
【摘要】 目的分析心脏彩超在冠心病节段性室壁运动异常患者诊断中的应用效果。方法便利选取2019年3月—2020年4月期间在该院就诊的冠心病节段性室壁运动异常患者108例,108例患者均行心脏彩超检查,在此基础上再行冠状动脉造影检查。比较两组诊断方式对患者左房内径、左室末径、主动脉内径、右房内径、右室内径等的检出率及其诊断准确性、敏感性和特异性。结果冠状动脉造影检查和心脏彩超检查对冠心病节段性室壁运动异常患者的左房内径检出率分别为27.78%和22.22%,差异无统计学意义(χ~2=0.437,P>0.05);左室末径检出率分为14.81%和16.67%,差异无统计学意义(χ~2=0.068,P>0.05);主动脉内径检出率分别为18.51%和12.96%,差异无统计学意义(χ~2=0.584,P>0.05);右房内径检出率分别为24.07%和14.81%,差异无统计学意义(χ~2=1.378,P>0.05);右室内径检出率检分别为14.81%和11.11%,差异无统计学意义(χ~2=0.319,P>0.05)。冠状动脉造影检查和心脏彩超检查对冠心病节段性室壁运动异常患者的诊断准确性分别为100.00%和77.78%,差异有统计学意义(χ~2=13.473,P<0.05);诊断敏感性分别为98.14%和79.62%,差异有统计学意义(χ~2=9.284,P<0.05);诊断特异性分别为100.00%和79.62%,差异有统计学意义(χ~2=12.195,P<0.05)。结论心脏彩超诊断冠心病节段性室壁运动异常虽然相比冠状动脉造影的诊断准确性、敏感性和特异性较低,但是其总体诊断准确性仍较高,对患者的各项临床指标检出率均与冠状动脉造影检验相当,且心脏彩超具有无创、操作简单,费用低廉、可重复进行等优势,值得在临床进行推广应用。
【Abstract】 Objective To analyze the application effect of cardiac color Doppler ultrasound in the diagnosis of patients with segmental ventricular wall motion abnormalities in coronary heart disease. Methods From March 2019 to April 2020, 108 patients with coronary artery disease and segmental wall motion abnormalities who were treated in the hospital were conveniently selected and all underwent cardiac color Doppler ultrasound examination. On this basis, coronary angiography was performed again. The detection rates of left atrial diameter, left ventricular diameter, aortic diameter, right atrial diameter, right ventricular diameter, etc., and the diagnostic accuracy, sensitivity and specificity of the two groups of diagnostic methods were compared. Results The detection rates of coronary angiography and cardiac color Doppler ultrasound in patients with segmental ventricular wall motion abnormalities in coronary heart disease were 27.78% and22.22%, respectively,the difference was not statistically significant(χ~2=0.437, P>0.05). The detection rate of left ventricular diameter was 14.81% and 16.67%, the difference was not statistically significant(χ~2=0.068, P>0.05); the detection rate of aortic inner diameter was 18.51% and 12.96%, the difference was not statistically significant(χ~2=0.584, P >0.05); the detection rate of the right atrium diameter was 24.07% and 14.81%, the difference was not statistically significant(χ~2=1.378,P>0.05); the detection rate of the right ventricular diameter was 14.81% and 11.11%, the difference was not statistically significant(χ~2=0.319, P>0.05). The diagnostic accuracy of coronary angiography and color echocardiography for patients with segmental ventricular wall motion abnormalities in coronary heart disease were 100.00% and 77.78%, respectively, the difference was statistically significant(χ~2=13.473, P<0.05); the diagnostic sensitivity was respectively 98.14% and 79.62%,the difference was statistically significant(χ~2=9.284, P <0.05); the diagnostic specificity was 100.00% and 79.62%,the difference was statistically significant respectively(χ~2=12.195, P <0.05). Conclusion Although the diagnostic accuracy,sensitivity and specificity of cardiac color Doppler ultrasound in the diagnosis of segmental ventricular wall motionabnormalities in coronary heart disease is lower than that of coronary angiography, its overall diagnostic accuracy isstill higher, which is useful for various clinical indicators of patients. The detection rate is comparable to that of coronary angiography, and it has the advantages of cardiac color Doppler ultrasound, which is non-invasive, simple to operate, low cost, and reproducible, and it is worthy of clinical application.
