血清ApoE、LP-a及Lp-PLA2水平在冠心病预测中的应用价值
Application Value of Serum ApoE,LP-a and Lp-PLA2 Levels in the Prediction of Coronary Heart Disease
【摘要】 目的探究血清ApoE、LP-a及Lp-PLA2水平在冠心病预测中的应用价值。方法从该院2017年12月—2019年12月方便选取130例冠心病患者,作为观察组,同期选取60名进行体检的健康志愿者,作为对照组。检测比较观察组和对照组的血清ApoE、LP-a及Lp-PLA2水平,评估其对冠心病的预测性能。结果观察组患者的血清ApoE、LP-a及Lp-PLA2水平为(41.15±3.01)mg/L、(289.43±12.27)mg/L、(301.21±16.73)ng/mL明显高于对照组的(36.58±2.76) mg/L、(135.15±9.52)mg/L、(163.96±13.87)ng/m L,差异有统计学意义(t=9.981、86.121、55.350,P<0.05);轻度、中度、重度冠脉狭窄患者的血清ApoE(38.63±2.04)mg/L、(40.51±2.18)mg/L、(42.07±2.93)mg/L、LP-a(173.68±8.72)mg/L、(205.93±10.45)mg/L、(290.86±11.76)mg/L及Lp-PLA2(183.25±12.07)ng/mL、(225.89±9.56)ng/mL、(312.43±10.22)ng/mL,经单因素方差分析,数据差异有统计学意义(F=22.339、1 471.995、1 633.116,P<0.05);Lp-PLA2的曲线下面积(AUC)最高,特异度93.76%,灵敏度为84.15%,诊断准确性最高,在冠心病预测中的价值最优。结论血清ApoE、LP-a及Lp-PLA2水平对冠心病的预测有重要的临床价值,尤其是Lp-PLA2指标,3项指标与冠心病呈现正相关关系,可以作为预测标志物。
【Abstract】 Objective To explore the application value of serum ApoE,LP-a and Lp-PLA2 levels in the prediction of coronary heart disease.Methods From December 2017 to December 2019 in our hospital,130 patients with coronary heart disease were selected as the observation group,and 60 healthy volunteers who underwent physical examination were selected as the control group.Detect and compare the serum ApoE,LP-a and Lp-PLA2 levels of the observation group and the control group to evaluate their predictive performance for coronary heart disease.Results The serum ApoE,LP-a and Lp-PLA2 levels of the observation group were(41.15 ±3.01)mg/L,(289.43 ±12.27)mg/L,(301.21 ±16.73)ng/mL,which were significantly higher than those of the control group(36.58±2.76)mg/L,(135.15±9.52)mg/L,(163.96±13.87)ng/mL,the difference was statistically significant(t =9.981,86.121,55.350,P <0.05);serum ApoE in patients with mild,moderate,and severe coronary stenosis(38.63 ±2.04)mg/L,(40.51 ±2.18)mg/L,(42.07 ±2.93)mg/L,LP-a(173.68 ±8.72)mg/L,(205.93 ±10.45)mg/L,(290.86±11.76) mg/L and Lp-PLA2(183.25±12.07)ng/mL,(225.89±9.56)ng/mL,(312.43±10.22)ng/mL,One-way analysis of variance,the difference was statistically significant(F=22.339,1471.995,1633.116,P<0.05);Lp-PLA2 had the highest area under the curve(AUC),with a specificity of 93.76%,a sensitivity of 84.15%,and diagnostic accuracy was the highest,the best value in the prediction of coronary heart disease.Conclusion Serum levels of ApoE,LP-a and Lp-PLA2 have important clinical value for the prediction of coronary heart disease,especially Lp-PLA2 index.The three indexes have a positive correlation with coronary heart disease and can be used as predictive markers.
