STA-MCA联合EDMS血管重建手术对烟雾病的疗效
The efficacy of STA-MCA combined with EDMS vascular reconstruction surgery in the treatment of moyamoya disease
【摘要】 目的探究颞浅动脉-大脑中动脉分支吻合术(STA-MCA)联合脑-硬脑膜-肌肉-血管融合术(EDMS)治疗烟雾病(MMD)的效果及其对脑血流状态的影响。方法选取MMD患者87例,根据随机数字表法分组。对照组43例给予脑-硬脑膜-动脉-肌肉血管融合术(EDAMS)治疗,观察组44例给予STA-MCA联合EDMS治疗,对比疗效。结果手术后观察组TTP水平、m RS评分低于对照组,CBF、脉压、血流峰值、总有效率高于对照组(P<0.05);两组术后并发症比较,差异无统计学意义(P>0.05)。结论 STA-MCA联合EDMS,可显著改善MMD患者脑血流动力学,且安全性高。
【Abstract】 Objective To explore the effect of superficial temporal artery-middle cerebral artery branch anastomosis(STA-MCA) combined with brain-dural-muscle-vascular fusion(EDMS) in the treatment of moyamoya disease(MMD)and its effect on cerebral blood flow.Methods 87 patients with MMD were selected and randomly divided into two groups. 43 cases in the control group were treated with EDAMS, and 44 cases in the observation group were treated with STA-MCA and EDAMS. The curative effect was compared. Results After the operation, the TTP level and mRS score of the observation group were lower than those of the control group; the CBF, pulse pressure, blood flow peak value and total effective rate were higher than those of the control group(P<0.05); there was no significant difference in postoperative complications between the two groups(P>0.05). Conclusion STA-MCA combined with EDMS can significantly improve cerebral hemodynamics in patients with MMD, which has high safety.
