【摘要】 目的研究老年听力障碍筛查量表(HHIE-S)应用于社区60岁以上老年人听力筛查的价值,讨论其与纯音听阈测试的相关性,评估HHIE-S应用于大规模听力筛查的可行性。方法对北京市海淀区346例老年人进行纯音听阈测试及HHIE-S问卷调查,计算平均听阈及HHIE-S量表得分,将得分>8分定义为存在听力障碍,分析HHIE-S应用在老年人大规模听力筛查中的可行性,计算该量表对老年人不同程度听力损失的敏感性、特异性、阳性预测值,探讨HHIE-S量表与纯音听阈测试的相关性。结果 HHIE-S量表对于轻度、中度、重度听力损失的敏感性分别为43.6%、67.05%、94.12%,特异性分别为97.04%、85.66%、75.68%,阳性预测值分别为98.6%、44.7%和9.2%。HHIE-S量表与老年人听力损失程度相关系数分别为0.443、0.513、0.337(P<0.001)。结论 HHIE-S量表与纯音听阈测试有很好的相关性,在中度及以上听力损失敏感性较高,可用于筛查老年人的听力损失。更多还原

【Abstract】 Objective To determine and evaluate the application value of the hearing handicap inventory for the elderly-screening version(HHIE-S) in community-based hearing screening for the elderly over 60 years old, to discuss its correlation with the pure-tone audiometry, and to assess the feasibility of applying the HHIE-S scale in mass hearing screening.Methods The pure tone audiometry and HHIE-S questionnaire were performed on 346 elderly people in Haidian District, Beijing City and the average hearing threshold and HHIE-S scale scores were calculated, in which a scale score higher than 8 was defined as the presence of hearing impairment. The feasibility of applying the HHIE-S scale in mass hearing screening of the elderly was analyzed. The sensitivity, specificity and positive predictive value of the scale for different degrees of hearing loss in the elderly were calculated, and the correlation between the HHIE-S and the pure tone audiometry was explored.Results The sensitivity of the HHIE-S scale for mild, moderate and severe hearing loss were 43.6%, 67.05% and 94.12%, respectively. The specificity were 97.04%, 85.66% and 75.68%, respectively. The positive predictive values were 98.6%, 44.7% and 9.2%, respectively. The correlation coefficients between the HHIE-S scale and the degree of hearing loss in the elderly were 0.443, 0.513, 0.337, respectively(P<0.001).Conclusion The HHIE-S scale has good consistency between the score and the pure tone threshold, and it has high sensitivity for moderate and above hearing loss, can be used to screen the elderly for hearing loss.更多还原
