踝关节骨折术后关节功能恢复的影响因素分析

【摘要】 目的探究踝关节骨折术后关节功能恢复的影响因素及其康复护理措施。方法选择医院2017年1月至2019年8月收治的踝关节骨折手术患者100例,通过回顾分析获取其基础信息,借助Mazur踝关节评价分级系统评估术后关节功能恢复状况,设立术后关节功能恢复可、差组患者24例,术后关节功能恢复优、良组患者76例。基于单因素分析、多因素Logistic回归分析明确术后关节功能恢复的影响因素,并确定康复护理对策。结果单因素分析显示,术后关节功能恢复的相关因素有年龄、骨折分型、跟骨牵引、早期功能锻炼、手术方式、送治时间、内固定物未取出、术后并发症、术后疼痛VAS评分(P<0.05);多因素Logistic回归分析显示,术后关节功能恢复的独立影响因素有年龄≥50岁、C型骨折、未做跟骨牵引、早期功能锻炼、内固定物未取出、术后并发症、VAS疼痛≥4分(P<0.05)。结论踝关节骨折术后关节功能恢复受多因素影响,有年龄≥50岁、C型骨折、未做跟骨牵引、早期功能锻炼、内固定物未取出、术后并发症、VAS疼痛≥4分,需加强术后并发症防控,积极开展早期功能锻炼,以促进踝关节功能恢复。

【Abstract】 Objective To explore the factors affecting the recovery of joint function after undergoing ankle fracture surgery and their rehabilitation nursing measures. Methods A total of 100 patients undergoing ankle fracture surgery in the hospital from January 2017 to August 2019 were selected, and the basic information was obtained through retrospective analysis, and the postoperative joint function recovery was assessed by the Mazur ankle joint evaluation grading system to set a control group with 24 cases of patients and an excellent groups with 76 cases of patients for studying postoperative joint function. Based on single factor analysis and multi-factor Logistic regression analysis, the influencing factors of postoperative joint function recovery were identified, and rehabilitation nursing strategies were determined. Results Univariate analysis showed that the factors related to the recovery of joint function after operation included age, fracture classification, calcaneal traction, early functional exercise, operation method, treatment time, internal fixation not removed, postoperative complications, postoperative pain VAS score(P <0.05). Multivariate Logistic regression analysis showed that the independent influencing factors of joint function recovery after operation included age ≥ 50 years old, type C fracture, no calcaneal traction, early functional exercise, internal fixation not removed, postoperative complications, VAS score ≥ 4 points(P<0.05). Conclusion The recovery of joint function after ankle fracture was affected by multiple factors, including age ≥ 50 years old, type C fracture, no calcaneal traction, early functional exercise, internal fixation not removed, postoperative complications, VAS score ≥ 4 points. It was necessary to strengthen the prevention and control of postoperative complications, and actively carry out early functional exercises to promote the recovery of ankle joint function.
