磁共振功能成像对复方生脉成骨胶囊治疗肾虚血瘀型早期激素性股骨头坏死的评价分析

【摘要】 目的通过磁共振功能成像评价复方生脉成骨胶囊治疗肾虚血瘀型早期激素性股骨头坏死的疗效。方法将40例肾虚血瘀型早期激素性股骨头坏死患者按照随机数字表法分为2组,对照组20例予西医常规治疗,治疗组20例在对照组基础上加用复方生脉成骨胶囊治疗。2组均治疗3个月,治疗前后均行磁共振功能成像分析[水脂分离成像(Dixon)及体素内不相干运动扩散加权成像(IVIM-DWI)],比较2组感兴趣区(ROI)微循环灌注分数(f)值及脂肪分数(FF)值变化情况。结果与治疗前比较,治疗组治疗后ROI微循环灌注f值明显升高(P <0.05),脂肪含量FF值明显降低(P <0.05),且均优于对照组治疗后(P <0.05)。结论复方生脉成骨胶囊治疗肾虚血瘀型早期激素性股骨头坏死疗效确切,可明显改善股骨头坏死区域微循环,减轻骨髓脂肪化,从而达到促进坏死骨组织生长修复的作用,磁共振功能成像技术可作为疗效评估的影像依据,具有准确客观的优势。
【Abstract】 Objective To analyze the effect of Fufang Chenggu Capsule in treatment of hormone-induced early femoral head necrosis of kidney deficiency and blood stasis by functional magnetic resonance imaging( f MRI). Methods Forty patients with hormone-induced early femoral head necrosis were randomly divided into the control group( n = 20) and the treatment group( n = 20). The control group was treated with conventional western medicine treatment,the treatment group was additionally treated with compound shengmai chenggu capsule on the basis of the control group,and both groups were treated for 3 months.Water and fat separation technique( Dixon) and intravoxel incoherent motion diffusion weighted imaging( IVIM-DWI)) were performing before and after treatment in the two groups. Microcirculation perfusion fraction( f) value and fat fraction( FF)value of region of interest( ROI) were compared between the two groups. Results Compared with before treatment,f-value of RIO was significantly increased and FF-value of RIO was significantly lower in the two groups after treatmeat( all P < 0. 05),which in treatment group were all better than the control group( P < 0. 05). Conclusion When patient with hormone-induced early femoral head necrosis is concurrent with kidney deficiency and blood stasis,using Fufang Shengmai Chenggu Capsule has a definite therapeutic effect,which can significantly improve the microcirculation of the ROI,reduce the fatification of the bone marrow,So as to promote the growth and repair. FMRI technology has accurate and objective advantages when it is used as an imaging basis for evaluating the efficacy.
