【摘要】 目的观察红元胶囊联合利伐沙班片对老年股骨粗隆间骨折术后患者下肢深静脉血栓形成(DVT)的影响。方法将98例老年股骨粗隆间骨折术后患者按照随机数字表法分为2组,对照组49例术后6 h予利伐沙班片治疗;治疗组49例在对照组治疗基础上加用红元胶囊治疗。2组均治疗2周,出院后随访至术后1个月。比较2组术后1个月下肢DVT发生率;比较2组治疗前后下肢周径差;比较2组术前及术后3、7 d活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、纤维蛋白原(FIB)水平变化;比较2组不良反应发生率。结果治疗组下肢DVT发生率8.16%(4/49),对照组下肢DVT发生率20.41%(10/49),治疗组下肢DVT发生率低于对照组(P <0.05)。2组治疗后大腿周径差、小腿周径差均较本组治疗前减小(P <0.05);治疗组治疗后大腿周径差、小腿周径差均小于对照组(P <0.05)。对照组术后3、7 d FIB水平均较本组术前升高(P <0.05);术后3、7 d对照组FIB水平均高于治疗组(P <0.05)。结论老年股骨粗隆间骨折术后患者应用红元胶囊联合利伐沙班片治疗,有助于预防下肢DVT,改善血液高凝状态。更多还原

【Abstract】 Objective Objective To observe the clinical influence of hongyuan capsule combined with rivaroxaban tablets on lower extremity deep venous thrombosis( DVT) of elderly postoperative patients with femoral intertrochanteric fractures( FIF). Methods Ninety-eight elderly postoperative patients with FIF were randomly divided into the control group( 49 cases) and the treatment group( 49 cases). The control group was treated with rivaroxaban tablets 6 hours after the operation,and the treatment group was treated with hongyuan capsule on the basis of the control group. treatment for 2 weeks,incidence of lower extremity DVT were compared in the two groups one month after surgery; lower extremity circumference difference were compared in the two groups before and after treatment; blood coagulation function including activated partial thromboplastin time( APTT),prothrombin time( PT) and fibrinogen( FIB) level were compared in the two groups before operation and 3,7 days after operation,as well as incidence of adverse reactions was recorded. Results The incidence of lower extremity DVT in the treatment group was lower than that in the control group [8. 16%( 4/49) vs 20. 40%( 10/49),( P < 0. 05),respectively]. Compared with before treatment,the thigh circumference difference and calf circumference difference in both groups was decreased( P <0. 05),which was lower in treatment group than control group( P < 0. 05). Compared with before operation,FIB level in the control group was increased at 3 and 7 days after operation( P < 0. 05),which was higher in the control group than the treatment group at same period( P < 0. 05). Conclusions Hongyuan capsule combined with rivaroxaban tablets for elderly postoperative patients with FIF can prevent lower extremity DVT and improve blood hypercoagulability.更多还原
