血清维生素A、E水平及细胞免疫指标与儿童反复呼吸道感染相关性研究

【摘要】 目的探讨血清维生素A、E水平及细胞免疫指标与儿童反复呼吸道感染(RRTI)的相关性。方法选取2018年7月至2020年7月收治的RRTI儿童2 984例作为RRTI组,同期非RRTI儿童4 012例作为对照组。比较2组血清维生素A、维生素E、细胞免疫指标(CD+3、CD+4、CD+4/CD+8);分析RRTI患儿血清维生素A、E与细胞免疫指标的相关性;分析维生素A、E及细胞免疫指标对RRTI的影响。结果 RRTI组血清维生素A、E及CD+3、CD+4、CD+4/CD+8水平为(0.25±0.07)mg/L、(7.13±2.33)mg/L、(63.57±4.21)%、(34.61±3.98)%、1.23±0.19,均低于对照组[(0.36±0.08)mg/L、(8.94±1.93)mg/L、(67.23±4.94)%、(40.19±3.82)%、1.51±0.23],差异有统计学意义(P<0.05);RRTI组维生素A、E缺乏率25.50%、9.01%,为高于对照组(2.67%、0.45%),差异有统计学意义(P<0.05)。Pearson相关分析显示,RRTI患儿血清维生素A与CD+3、CD+4、CD+4/CD+8均呈正相关(r=0.222、0.340、0.310,P<0.05),血清维生素E与CD+3、CD+4、CD+4/CD+8均呈正相关(r=0.139、0.234、0.201,P<0.05)。Logistic回归分析显示:维生素A、E缺乏是儿童RRTI的危险因素(Or=13.840、28.618,95%CI=10.292~18.611、15.091~54.270,P<0.05),而CD+3、CD+4、CD+4/CD+8是RRTI的保护性因素(OR=0.846、0.692、0.002,95%CI=0.831~0.861、0.676~0.708、0.001~0.003,P<0.05)。结论 RRTI患儿血清维生素A、E含量相对缺乏,机体细胞免疫功能失衡,且维生素A、E及CD+3、CD+4、CD+4/CD+8与儿童RRTI的发生发展密切相关。

【Abstract】 Objective To investigate the correlation of serum vitamin A, vitamin E levels and cellular immune indicators with recurrent respiratory tract infections(RRTI) in children.Methods A total of 2,984 chidren patients with RRTI who were treated in our hospital from July 2018 to July 2020 were enrolled as RRTI group, at the same time, the other 4,012 chidren without RRTI were enrolled as control group. The serum levels of vitamin A, vitamin E, and cellular immune indexes(CD+3,CD+4,CD+4/CD+8) were observed and compared between the two groups. The correlation between serum vitamin A, vitamin E and cellular immune indexes as well as the effects of vitamin A, vitamin E and cellular immune indexes on RRTI were analyzed.Results The serum levels of vitamin A, vitamin E, CD+3、CD+4、CD+4/CD+8 in RRTI group were significantly lower than those in control group(P<0.05). The deficiency rates of vitamin A and E in RRTI group were significantly higher than those in control group(P<0.05). Pearson correlation analysis showed that serum vitamin A and serum vitamin E were positively correlated with CD+3,CD+4,CD+4/CD+8(P<0.05). Logistic regression analysis showed that vitamin A and E deficiencies were risk factors of RRTI(P<0.05), however, CD+3,CD+4,CD+4/CD+8 were the protective factors of RRTI(P<0.05).Conclusion The contents of serum vitamin A and E are relatively insufficient in children with RRTI,with imbalance of body’s cellular immune function. In addition, vitamin A, vitamin E, CD+3,CD+4,CD+4/CD+8 are closely correlated with the occurrence and development of RRTI in children.
