【摘要】 目的分析经皮椎体后凸成形术(PKP)对骨质疏松性椎体压缩性骨折(OVCF)患者的临床应用价值。方法收集2018年6月—2020年6月收治的OVCF 102例的临床资料。根据治疗方法分为A组52例和B组50例。A组给予PKP治疗,B组给予经皮椎体成形术(PVP)治疗。比较2组围手术期指标、影像学参数、视觉模拟评分法(VAS)、Oswestry功能障碍指数、生活质量及并发症发生率。结果 A组手术时间、住院时间均显著长于B组(P<0.05)。与术前比较,术后7 d, 2组椎体前缘、中部、后缘高度及生活质量评分明显上升,VAS评分、Oswestry功能障碍指数降低,且上述指标A组改善幅度优于B组(P<0.05,P<0.01)。2组术后7 d伤椎局部Cobb角低于术前(P<0.05)。2组术后7 d伤椎局部Cobb角及并发症总发生率组间比较差异无统计学意义(P>0.05)。结论 PKP治疗OVCF可有效缓解疼痛程度,改善肢体功能并提高生活质量,且骨水泥渗漏率低。更多还原

【Abstract】 Objective To analyze the clinical value of percutaneous kyphoplasty(PKP) in treatment of patients with osteoporotic vertebral compression fracture(OVCF). Methods Clinical data of 102 patients with OVCF admitted between June 2018 and June 2020 was collected, and the patients were divided into group A(n=52) and group B(n=50) according to different therapeutic methods. Group A was treated with PKP, while group B was treated with percutaneous vertebroplasty(PVP). The perioperative indicators, imaging parameters, visual analogue scales(VAS) score, Oswestry dysfunction indexes(ODI), quality of life, and incidence rate of complications were compared between two groups. Results Operative duration and length of hospitalization were significantly longer in group A than those in group B(P<0.05). After operation for 7 d, heights of the anterior, middle and posterior edges of the vertebrae as well as scores of quality of life were significantly higher, while VAS scores and ODI values were significantly lower than those before operation in two groups; the improvement degrees of the above indicators in group A were better than those in group B(P<0.05, P<0.01). Values of local Cobb angle of the injured vertebrae after operation for 7 d were significantly lower than those before operation in both groups(P<0.05). There were no significant differences in local Cobb angle of the injured vertebrae and the total incidence rate of complications after operation for 7 d between two groups(P>0.05). Conclusion PKP may effectively relieve pain and improve limb function and quality of life with lower leakage rate of bone cement in treatment of patients with OVCF.更多还原
