虫类药联合贝前列素治疗下肢动脉硬化闭塞症效果及安全性观察
Clinical Efficacy and Safety of Insect Drugs Combined with Beraprost in Treatment of Patients with Lower Extremity Arteriosclerosis Obliterans
【摘要】 目的探讨虫类药联合贝前列素治疗下肢动脉硬化闭塞症(ASO)的效果及安全性。方法选取2019年11月—2020年11月就诊于石家庄市中医院脉管科的90例ASO,根据治疗方法的不同分为观察组和对照组各45例。对照组在基础治疗上加用贝前列素40μg每日3次口服,观察组在对照组基础上联合虫类药口服,2组均治疗4周。观察2组临床疗效,治疗前后踝肱指数(ABI)、间歇性跛行距离、腘动脉最大血流速度、足背动脉血流量、胫后动脉血流量和血脂、炎性因子水平,以及不良反应发生情况。结果治疗后观察组临床总有效率明显高于对照组(P<0.05)。治疗后,2组ABI、间歇性跛行距离、腘动脉最大血流速度、足背动脉血流量、胫后动脉血流量、低密度脂蛋白胆固醇、总胆固醇、超敏C反应蛋白、白细胞介素-6较治疗前改善,且观察组上述指标优于对照组,差异有统计学意义(P<0.05)。治疗期间,2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论虫类药联合贝前列素治疗可改善ASO患者血脂情况,降低炎性因子水平,增加下肢血流量,延长间歇性跛行距离,具有较好的临床效果及安全性。
【Abstract】 Objective To investigate clinical efficacy and safety of insect drugs combined with Beraprost in treatment of patients with lower extremity arteriosclerosis obliterans(ASO). Methods A total of 90 patients with lower extremity ASO treated between November 2019 and November 2020 were selected and divided into observation group(n=45) and control group(n=45) according to different treatment methods. Control group was supplemented with 40 μg oral Beraprost(3 times a day) on the basis of basic treatment, while observation group was given oral insect drugs on the basis of treatment for control group. The two groups were all treated for 4 weeks. Clinical efficacy, ankle brachial index(ABI), intermittent lameness distance, popliteal artery maximum blood flow velocity, blood flow volume of dorsal pedis artery, blood flow volume of posterior tibial artery, blood lipid, levels of inflammatory factor and incidence rates of adverse reactions were observed in two groups. Results The total effective rate in observation group was significantly higher than that in control group(P<0.05). After treatment, levels of ABI, intermittent lameness distance, popliteal artery maximum blood flow velocity, blood flow of dorsal pedis artery, blood flow volume of posterior tibial artery, low density lipoprotein cholesterol, total cholesterol, hypersensitive C reactive protein and interleukin-6 were significantly improved compared with those before treatment in two groups, and the above levels in observation group were more significantly better compared with those in control group(P<0.05). During the treatment, there was no significant difference in the total incidence rate of adverse reactions between two groups(P>0.05). Conclusion The combination of insect drugs and Beraprost may improve blood lipid, reduce levels of inflammatory factors, increase blood flow volume of lower extremity and prolong intermittent limp distance with good clinical efficacy and safety in treatment of patients with lower extremity ASO.
