儿童与成人新型冠状病毒肺炎的CT影像学分析

【摘要】 目的探讨家庭聚集性发病的新型冠状病毒肺炎(COVID-19)中儿童与成人CT影像学改变情况。方法回顾性分析2020年1月21日至2月6日在中山大学附属第五医院住院的5个家庭聚集性发病的18例COVID-19患者的CT影像学资料。结果 18例患者中儿童5例,男性4例,女性1例,平均年龄(2.83±2.02)岁;成人13例,男性6例,女性7例,平均年龄(46.77±12.49)岁;5例患儿确诊时(初诊)胸部CT有1例未见明显病灶,2例病变累及左肺下叶,2例病变累及双肺;13例成人确诊时(初诊)胸部CT有1例未见明显病灶,5例累及单侧肺部,7例累及双肺,两者间比较差异无统计学意义(P>0.05);治疗3~5 d后复查胸部CT,5例儿童均好转,13例成人中,9例加重,2例好转,2例无改变,两者之间差异具有统计学意义(P<0.05)。结论家庭聚集性发病COVID-19患者中,儿童CT影像学改变普遍比成人轻,3~5 d后复查儿童处于恢复期,而成人大部分处于进展期。

【Abstract】 Objective To analyze the characteristics of CT imaging in children and adults with familial cluster cases of coronavirus disease 2019(COVID-19). Methods CT imaging changes from 18 patients with COVID-19, who were from five family clusters and admitted to the Fifth Affiliated Hospital of Sun Yat-sen University from January 21 to February 6, 2020 were retrospectively analyzed. Results The 18 patients with COVID-19 included five children(four males and one female), with an average age of(2.83 ± 2.02) years. There were thirteen adults(six males and seven females), with an average age of(46.77±12.49) years. In the five children, one patient had no obvious lesion on initial chest CT, two patients had lesions involving the lower lobe of the left lung, and two patients had lesions involving both lungs.One of the thirteen adults had no obvious lesion in chest CT, five patients had unilateral lung involvement, and seven patients had bilateral lung involvement. There was no significant difference between children and adults at the initial chest CT imaging(P>0.05). After 3 to 5 days of treatment, chest CT was reexamined. All children were improved, in the thirteen cases of adults, nine cases were aggravated, two cases were improved, two cases had no change. There was a statistically significant difference between children and adults in CT reexamination(P<0.05). Conclusion Among COVID-19 patients from family clusters, CT imaging changes in children are generally lighter than adults. After 3 to 5 days, the majority of children were in the recovery stage, while the majority of adults were in the progressive stage in CT imaging.
