25羟维生素D与中国人群糖尿病肾病进展风险相关性的Meta分析

【摘要】 目的系统评价中国人群血清25(OH)D水平和2型糖尿病肾病(T2DN)进展风险的关系。方法计算机检索Pub Med、EMbase、中国知网数据库(CNKI)、重庆维普中文科技期刊数据库(VIP)、万方数据,检索时限均为建库至2019年10月30日,由二位研究员筛选确定病例对照研究文献,选取有效数据,评价文献质量,使用Stata12.0软件进行Meta分析,合并效应量以标准化均数SMD值及95%CI表示。结果共纳入13个符合条件的病例对照研究,共2 335例,糖尿病大量白蛋白尿(MA)组368例,糖尿病微量白蛋白尿(MI)组759例,单纯糖尿病(NO)组721例,健康对照(CN)组487例;与CN组和NO组比较,MI组的血清25(OH)D均显著降低(分别为SMD=-2.02,95%CI:-2.55~-1.48,P<0.01;和SMD=-0.66,95%CI:-0.77~-0.55,P<0.01);与CN组和NO组比较,MA组的血清25(OH)D亦显著降低(分别为SMD=-3.35,95%CI:-4.39~-2.31,P<0.01;和SMD=-2.08,95%CI:-2.63~-1.53,P<0.01);与MI组比较,MA组的血清25(OH)D显著降低(SMD=-1.24,95%CI:-1.60~-0.89,P<0.01)。结论中国人群T2DN患者的血清25(OH)D水平下降,低25(OH)D血症与T2DN的进展风险增加相关。

【Abstract】 Objective To systemically review the correlation between serum 25-hydroxyvitamin D(25(OH)D)and type 2 diabetic nephropathy(T2 DN) risk in Chinese population. Methods The Pub Med, EMbase, CNKI, VIP and Wanfang data published before 30 October 2019 were searched, and two reviewers screened literature according to the predetermined inclusion and exclusion criteria, extracted data, and assessed the methodological quality of included studies. Then meta-analysis was performed using Stata 12.0 software. Pooled data were expressed as standardized mean difference(SMD) and 95% confidence interval(CI). Results A total of 13 eligible case-control studies involving 2 335 cases were enrolled in this study, which including 368 cases in diabetic macroalbuminuria(MA) group, 759 cases in diabetic microalbuminuria(MI) group, 721 cases in diabetes mellitus(NO) groups, and 487 cases in healthy control(CN)group. The results of meta-analysis showed that the serum 25(OH)D level in patients of MI was significantly lower than that in CN group and NO group(SMD=-2.02, 95%CI-2.55 to-1.48, P<0.01 and SMD=-0.66, 95%CI-0.77 to-0.55, P<0.01, respectively). The serum 25(OH)D level of MA was significantly lower than that in CN group and in NO group(SMD=-3.35, 95%CI-4.39 to-2.31, P<0.01 and SMD=-2.08, 95%CI-2.63 to-1.53, P<0.01, respectively). The serum25(OH)D level of MA was significantly lower than that in MI group(SMD=-1.24, 95% CI-1.60 to-0.89, P<0.01).Conclusion The serum 25(OH)D level is decreased in Chinese population of T2 DN, and the decrease of serum 25(OH)D is associated with the increased risk of T2 DN in Chinese population.
