骨形态发生蛋白复合材料联合空心螺钉治疗急性股骨颈骨折的疗效分析
Analysis of bone morphogenetic protein composites combined with hollow screws in the treatment of acute femoral neck fractures
【摘要】 目的探讨空心螺钉结合骨形态发生蛋白 2 (BMP-2) 复合材料行闭合复位内固定治疗急性股骨颈骨折的临床疗效。方法本研究收集了 2016 年 1 月至 2018 年 12 月收治的 120 例急性股骨颈骨折患者并分为研究组和对照组。研究组为接受闭合复位内固定联合 BMP-2 复合材料治疗的共 58 例患者 (男 27 例,女 31 例;左 30 例,右 28 例),平均年龄为 45.2 岁。对照组共 62 例 (男 30 例,女 32 例;左 29 例,右 33 例),接受闭合复位内固定治疗,平均年龄为 43.7 岁。收集患者的一般临床资料及手术时间、Harris 评分和并发症发生情况。结果研究组平均随访 28.2 个月,其中有 3 例股骨头坏死,其中 1 例为骨不连。Harris 平均得分为 89.1。对照组平均随访 30.5 个月。两组之间的年龄,性别和随访时间相比,差异均无统计学意义(P > 0.05)。对照组的平均 Harris 评分为 80.6,显著低于研究组 (P < 0.05)。对照组股骨头坏死发生率和骨折骨不连的发生率分别为 12.5% (7 / 56) 和 10.7% (6 / 56),明显高于研究组,差异有统计学意义 (P < 0.05)。结论急性移位的股骨颈骨折可采用闭合复位内固定联合 BMP-2 复合材料进行治疗,并发症少。
【Abstract】 Objective To investigate the clinical effect of closed reduction and internal fixation with hollow screw combined with bone morphogenetic protein 2 (BMP-2) composite material in the treatment of acute femoral neck fractures.Methods This study collected 120 patients with acute femoral neck fractures admitted from January 2016 to December 2018.All were divided into research group and control group.Research group:n=58 (27 males and 31 females,30 left and 28 right);closed reduction and internal fixation combined with BMP-2 composite;average age 45.2 years.Control group:n=62 (30 males and 32 females,29 left and 33 right);closed reduction and internal fixation;average age 43.7 years.General clinical data,operation time,Harris score and complications were recorded.Results Research group was followed up for an average of 28.2 months (Table 2),including 3 cases of femoral head necrosis and 1 case of nonunion.Harris scored an average of 89.1.Control group was followed up for an average of 30.5 months.No significant differences were observed in age,gender and follow-up between the two groups (P >0.05).The average Harris score of the control group was 80.6,which was significantly lower than that of the research group (P < 0.05).The incidence of femoral head necrosis and fracture nonunion in the control group were 12.5%(7 / 56) and 10.7% (6 / 56),which were significantly higher than those in the research group (P < 0.05).Conclusions Closed reduction and internal fixation combined with BMP-2 were recommended in the treatment of acutely displaced femoral neck fractures with fewer complications.
